I missed that you jumped up a full unit instead of the 2.5 I'd suggested. Must admit I'm worried he looks worse on this dose than he did on 2.0 units. Since Blue is newly back on insulin, don't assume his insulin patterns will look like Eddie's. I'd try getting a later test today, maybe around +8? You need to try to figure out when his onset and nadir typically are.

Good luck at the vet today. Hope things get cleared up and cleared out!

Blue is in hospital again. At the primary vet it was determined that he really was quite constipated after an x-ray and so they called the hospital and I took him over for fluid treatment and possibly an enema. I'm upset and worried, although I know this is what he needs.

As for the insulin: At times I'm caught between a rock and a hard place. The recommendation here was to go to 2.5u from 2.u. The IM vet wanted 4u and gave me some attitude about not going to 4u. I took the middle ground thinking that although 3u is more than 2u, it's far from 4u. Among other things, the IM vet said, "I need to be kept in the loop before you make changes, so we can discuss them before they are initiated."

I really don't assume Blue's insulin patterns will look like Eddie's, not at all. The most he's ever been was 12u and that was before his SRT.

At least Blue is getting fluids and hopefully this can be resolved. His dentals are really bad: infection so he's also going to get an antibiotic.

This is hard.

Hopefully the enema will do the trick. Lots of cats here have had them done. Have you done fluids before? It’s a good tool to have, also for CKD. Maybe they will show you how if you haven’t done them before.

The antibiotics also a good idea, pulse antibiotics is something CKD kitties can do if bad teeth and cannot do surgery.

As for dose, you hold the syringe. I just told you what TR considers safe.

Constipation can be frustrating. Tubby was a tough case and dealing with constipation/obstipation was intense at times, so I really do understand how you feel. The good news, to my mind, is that Blue doesn’t have an ongoing history of constipation to date, he hadn’t reached a crisis point of projectile vomiting, open mouth breathing, and intense pain, and he’s in hospital where they have options to help him such as fluids, medications, enemas, manual extraction, X-rays to monitor status, and even surgery if needed.

The worst is always the early days when the train seems out of control. One step at a time…

Thanks, JL, the train does seem to be hurtling pell-mell down the tracks. Blue may be in hospital for 2 days. They think an enema would be ineffective given the amount lodged there at the end of the road. They are doing a procedure that involves the nasal passage and a small tube that can deliver what needs to dissolve the block of poo, I guess you could say. That way, they don't overwhelm the heart (if that were the case) with fluids as they can administer them through the tube and not through a catheter.

I feel sick, to be honest.
 
Blue.jpg Apparently, Blue has eaten and taken his medications. They are about to insert a nasal gastric tube and I feel like weeping. The vet said at least it's a very soft tube and they will put a little freezing on his nose every 4 hours but he has to wear a cone. My poor Blue.

I think it’s very good that they’re taking this seriously and moving straight to the procedure. So often it seems there’s a push to go with an enema (or three) and “wait and see,” only for things to get worse and trickier to resolve. But even though he’s in good hands, I know this is excruciating for you. Just know you’re not out there alone; I’ll be thinking of you both, hoping for “action” on Blue’s behalf (he’s gorgeous, btw) and looking for updates.

So, I just called the hospital. They said Blue took the little nasal tube "and he was really good" and he's resting easily as they very mildly sedated him. They are also giving him antibiotics for the awful teeth problem. They also said results from the tube and infusion of the PEG (? didn't have a pen but will get this tomorrow) usually takes about 8 to 10 hours and they will be massaging him to get things moving. I feel relief knowing he is in good hands.
