Well, just when I think things are better, Henry wakes me up with the most frail and weak meows I've ever heard him make. I know immediately there was a problem. He only ate a little bit of his overnight foods. He only ate about 1/3 of his breakfast. He begged for something else so I gave him another flavor which he turned his nose up to. Checked ketones and they're back up to 4.4 :banghead: I don't know how this can happen so fast :(

Just gave 50ml subQ fluids. He's drank about 100ml of water and he's still going. More interested in water than food. I wish I'd started the budesonide yesterday when his ketones were 0.5 but I was waiting for the vet to e-mail or call first. I should have just done it :( I'm going to have to start being more aggressive with the R to get his BG down. Seems like the only thing I can do at this point.

I do hope the R will help. Remind me - do you have any Ondansetron? Let’s hope the ketones will come down as fast as they went up!

Yes, they came down pretty quickly before. I think him eating has a lot to do with the ketones. I leave food out for him and he usually eats most of it, but he didn't touch it or the food I put out in the middle of the night (my night anyway), He did eat a little this morning and I gave him his favorite (rabbit) 2 hours later which he ate most of. I'm just going to keep trying to feed him every 2 hours and if he doesn't want to eat, I'm going to have to syringe feed him I guess. BG is currently coming down. When that trend stops he'll be getting more R. I've gotta keep him out of the 300s. No Ondansetron, but I do have some cerenia, which I'll give him right now.
 
Ugh, so I figured I may as well give him some budesonide hoping that would help get more nutrients into him, just looked at the medicine and they gave me someone elses medicine with the same last night of both me and my vet. The compounding pharmacy is now closed until monday :(
 
Ong. Good you looked. What did they give you? That could have been really bad.
Gabapentin in Honey 100mg/ml, so yeah. It wouldn't have been good from the sugar alone. I just e-mailed the pharmacy manager, hopefully he's watching his emails and calls me. They have excellent reviews and a ton of employees. No idea why they can't keep one person there on the weekends or have an emergency phone line.

Thank you Diane, I do really appreciate it. I'm scouring the net to find his home phone number. He has been eating every 2 hours, so that's good. I really don't want to have to syringe feed him, so that's good. BG is down to 272. I'm watching it like a hawk and will be giving some R if it starts getting high again. Paws crossed he can clear these ketones as quickly as last time.

So grateful you checked the meds. I hope you can get it sorted out because I really think the anti inflammatory effect could help him. Hang in there, John and keep us posted on ketones.

It's hard to say. He's not pacing the house so that's good I guess. He looks a bit sick when I pick him up and hold him. I can tell when he isn't going good by looking at him, something about the way he holds his eyes and his second set of lids will be more visible, so he definitely isn't feeling good, but he is eating and drinking, so that's good. He hasn't meowed since he woke me up, but I'd never heard him meow like that before. Sounded like he could barely get it out.

I found the pharmacy manager/co-owners cell phone and email address and left/sent a voicemail/message. Hopefully I get a call back soon.

I've had good success finding & contacting people through facebook or instagram - send a message. Phone numbers can be hard these days.

Well, it worked. Just got a call back. They're sending someone to my house with the budesonide thank God. He's still eating well. BG is in the mid 200's. Watching him very closely.
 
They just delivered the budesonide. The founder owner actually delivered it which is pretty amazing considering they have over 100 employees. Anyway, just gave him his first dose. Paws crossed his BG behaves itself.

Well, we're back in the blue at 158 which means probably ~130 on the nova. Appetite has increased and he's eaten almost all of everything I've given him, so I'm hopeful the ketones have gone down. I'll check them at PMPS and give more subq fluids if they aren't down significantly. I hope his BG doesn't go too much lower, we don't need any more bouncing! Would it makes sense to give some MC food when he get close to 100 to try and stop the bounce?

I just left them a good review. It was an honest mistake and they made it right quickly and refunded what I had paid. They do mail order and have couriers for local delivery too. I'm considering switching my dogs compounded med to them. edit: They're also licensed to serve Washington, Idaho and Alaska in addition to Oregon if anyone else wants to support a good family owned business. https://www.northwestcompounders.com/

So great news everyone! I just took his PMPS and ketones are down to 0.2!!! I think that's the lowest ketone reading I've recorded ever! What the heck is going on? up and down so quick? These strips are all from the same pack. Maybe my strategy of feeding him every 2 hours or less is what got them down so quickly along with the subq fluids. He's still looking a bit sick to me and acting a little lethargic, so I think I'll give him the other 50ml of the subq fluids anyway. He hasn't been drinking much today and the pH of the fluid should bring his blood pH up some if it acidified much from the ketones and provide some (likely) needed potassium. It seems like the ketones happen when he doesn't eat the food I leave him overnight or give him in the night, so it's like any long period of not eating makes the ketones go through the roof.

I'm 10 minutes past shoot time because while his BG isn't super low (99) it's still falling even after eating about 3tsp of some 22% trying to slow it down, but it didn't slow it down much, so the number is a little food influenced (30 minutes ago or so maybe 45), I just don't want it to get super low so I end up in the purple/red for the next 5 cycles. Is @Bron and Sheba (GA) or @Bandit's Mom around at this hour? Any input, should I keep waiting or just shoot?

Alright, he's starting to go back up now, so I just gave him his 1.75uL 40 minutes late. I hope we don't shoot up with the same momentum we came down with. Would be nice to get a break from the reds for a day or two.
 
It seems that some cats' ketone levels just do that, spike up one day, back down the next. Hendrick was similar, but not to the same heights as Henry. Hendrick would be .3 one day, 1.6 the next.
