Help - FLUTD flare up, BG rising after pm shot

Hi folks,
I'm at a loss of what to do for Nico. We went to the vet clinic near my house on Friday because his sugar went up to 320. They advised it's most likely cystitis, gave him an onsior shot, and sent us home with Onsior pills. We finished his third dose yesterday, and today has been a crap shoot. He's still eating, as he's on Cerenia too, but the corn silk and D-mannose supplements do not seem to be working. Is this normal? Should I give a token dose since his glucose is going so crazy? I'm trying not to freak out, but I am not used to seeing my boy in the 300's. I feel so helpless.
 
I’m not a Prozinc user. He’s been on .25 of Vetsulin for a long time so seems like you could shoot since he is eating. So I think it was good to shoot.
 
We reserve the 911 for medical emergencies. Is there an emergency? If not please remove it so we know everything is ok.

I would keep to your normal dose, especially since he has something going on (remember the DKA recipe - not enough insulin + inappetence + inflammation/infection).

His BG should start to come down soon here if the Onsior works.

He hasn't had any Onsior since Sunday. His glucose is continuing to rise, he's almost at 400 now.
 
Did they do a urinalysis? Most often the cause of cystitis is an infection, and since his BG is so high after finishing the Onsior, I suspect something else is still going on.

They did a urinalysis, but said they didn't see any bacteria. But they did give me Clavamox, so he's on that right now as well (though I'm curious if the Clavamox is not working.) They did not have the ability to perform a culture. I'm thinking I need to call around tomorrow to see if a vet hospital is open, he needs something else if this is still happening to him. This would be his third UTI in a year.

Cystitis can be painful though, but I used buprenorphine for my cystitis kitty. Are you adding lots of water to his food, that really helps to flush him out. I also used Cosequin/glucosamine chondroitin supplements.

How is Nico acting otherwise? Peeing OK?

Yes, he's actually acting fine! His normal self, peeing, pooping, eating well. I did start him on Cosequin once the Onsior was finished, along with corn silk.
I do add water to his food, at least I have been. He also has a fountain downstairs but I'm not sure if he's drank from it yet tonight. I would say most likely, as I scooped around 9pm and just checked the litter box - I see pee in his "spot" (yes my cat is weird and pees in the same spot every time.)
I am curious if he needs something like bup to help his pain and inflammation?
 
Oh the only other thing he is doing, that I've noticed is he licks his pee place a lot. He does strain a little as well when peeing, but he is still peeing normal. There's also some protein deposits in his pee, but they said that could be from the cystitis.
 
The licking is also a sign of cystitis, some pain. Cerenia also has some anti-inflammatory properties. My cat was also on Cartrophen, equivalent is Adequan in the US. Hyaluronic acid drops might also help. All those things help the bladder walls. If it is cystitis, Clavamox won't do anything for him. At one point my kitty was also on some relaxant, whose name I can't remember (been almost a decade).

Thank you so much! I think I'm going to call my regular vet tomorrow and see if she can see him soon, and if I can get some pain medication in the interim. He seems to do fine for a few days and then this happens. Does that sound about right for cystitis? I wish I would have gotten more info from the clinic on Friday, this is just scary!

You can give a probiotic, just has to be spaced out at least two hours from the antibiotic (I'm not sure about spacing out from the other things). S boulardii is good for diarrhea specifically assuming the diarrhea is only from the clavamox (it doesn't work if that cat has EPI, low B12, or diarrhea from hyperthyroidism). Start with 1/4 capsule once a day, can increase as needed.

Thanks Frost, ironically I have another kitty with ibd so I have Proviable and S. Bouliardi! I gave Nico some Proviable in the middle of the night, he still had loose stool but not horrible.
 
Quick update - sugar was still high through today. Spoke to my regular vet, she came and got a urine sample and is going to send it out for analysis. She's a bit upset they didn't do it when we went on Friday, and is suspicious that he might have a UTI that is not responding to Clavamox. She said while that can work great for general UTI's, if this is a resistant one we need a stronger/ more precise antibiotic. The good thing is he is still acting mostly normal, so we'll find out what the culture shows tomorrow!
 
Ok, no bacteria in the urine, no WBC or RBC, no ketones, no protein even. ?? Vet said she will give me a call soon, and we'll see where to go next. I'm still wondering if it's cystitis or pancreatitis. He's been acting normal despite high numbers, eating ok since we've been doing Cerenia tabs since Saturday.
