It's the 2nd full day for Alec on lantus, yesterday gave 1.75U, last night he had soft stools, so decreased the dose to 1.5U this morning. Please advise PM dose, or should I skip? Thanks!

Would you mind putting TR into the signature please so we can see what dosing method you are following please?
Let’s see what the PMPS is before deciding the dose.
If it is lower than 150 stall, dont feed and test again in 20 minutes. And post and ask for assistance.
Are you going to be able to monitor this pm cycle?
Do you have a hypo kit set up with some higher carb food and some honey or Karo.

168+11.5
feed him at +11.5, plan to inject 1.25U PM.
With Lantus you don’t feed 30 minutes before the dose.
You test/feed/shoot one after the other at PMPS.
Lantus onset is not for 2 hours so you are shooting that number, not the number to you are just getting now.
Also Lantus like consistency so if you are going to keep changing the dose, you will not get the depot stabilised and consistent BG numbers.

just tested at 11.5 168, fed him 1 FF pate, and he finished everything
I can monitor his PM cycle tonight, yes have hypo kit ready.

Did you see post 7?
Lantus is different to vetsulin and you don’t need to feed 1/2 hour earlier. So I would follow the suggestion I gave in post 7 to test/feed / shoot at AMPS and PMPS.
As you are following TR I would stay with 1.5 units as long as you can monitor. That means getting a +1 and a +2 to start with (as you are shooting a lower preshot than normal) and then you may have to test during the whole cycle if he drops lower. I would give him a small snack at +2 +4 and +6 and then see how he is going . Always keep testing until the BG numbers mpstart to come back up and you know he has passed nadir (lowest point in the cycle)
How does that sound?

First time have a lower than 200 PMPS, so nervous about the next.
The final decision of how much insulin you give is up to you. You need to be comfortable with the decision.
But what ever you give , you are going to have to monitor as I suggested in post 10 as this cycle will be influenced by the depot of the previous cycle, which is how Lantus works.

Thanks, gave 1.5, and will monitor PM.

Also thanks for sharing about how to test/feed/shot one after another for lantus, this is new to me. Did not know that when I read TR dosing method. This is very helpful.

It can be confusing when swapping to another insulin.:)

Thanks, gave 1.5, and will monitor PM.
I will be around for the next 12 hours so will watch out for your posts.
Change your subject line to PMPS 168 and then add the +1 +2 etc so we can see. You can also post it in the thread.
Getting tests in early in the cycle will give an indication of what Alec has in mind.
He is responding nicely to Lantus!
