Taz earned a reduction last Friday but I didn't take it. I had been thinking about increasing his dose and I didn't feel like a decrease would have been in his favor. His numbers are normally higher than I'd like, and I feel like Friday's low was a fluke.

Anyway, he's been at 1.75 units for several weeks and I'm at a point where I don't know what to do. I don't want him to get insulin toxicity. Do I increase, decrease, or stay put?

I don't think I can do TR because I have an obsession with poking him, so that I can know where his glucose is constantly. I have promised myself (and him) that I won't test him more than 4-5 times daily, unless he is dropping quickly or is lower than normal. I don't think TR would be a good choice as I'd have to know how it was affecting him nonstop.

Also, his +1 and +2 are almost always lower than his preshot numbers. Why is this?

I highly recommend TR, it is good that you can check his BG without issue and TR is a more aggressive approach which is just what you need imo, based on how long you've been at 1.75IU

just another newbie opinion

It's the beginning of his cycle that I'm worried about if I switch to TR, because he usually drops his lowest in the first 4 hours. I usually work during that time, but I'm currently on break. Still, I'm not always home or in the house (I live on a farm with horses) during that time. :(

You would feed higher carbs to slow him down. What is your reduction number now with SLGS? I’d add that to your ss and signature.

Is that only if I'm following TR? I have started giving him a 1/2 tsp MC at his +1 again, to try to level him out. It didn't seem to work too well last time, so I stopped doing it.

Up until this last low, I reduce when he drops under 90.

No. If following TR you reduce if under 50. With SLGS you can lower your reduction number should you decide to continue with that method.

Truthfully I'm scared of the numbers allowed with TR. If the normal glucose for a non-diabetic cat is 80-120, I'd like to get to (and stay in) that range as much as possible.

With TR I would increase him by .25 every 3 days? What would be the goal numbers to reach, or how would I know when he was at a good dose and not to increase anymore?

With TR and a pet meter reductions under 68. Increase are every 6-10 cycles. You need to hold a dose for 7 days with SLGS. If the numbers climb clinb a after a reduction you still wait 7 days to increase if needed. With TR if a reduction fails you can go back up sooner. If you don’t feel comfortable with that stick with SLGS and take reductions at 80. Just add that to your signature.

I think I averaged around 5 pokes a day with Neko on TR, once I had a handle on what her cycles looked like. Of course more when dropping or lower. There is nothing about TR that says you have to be a testaholic. Learn your cat's reaction to insulin (onset, nadir, duration) and reaction to carbs/food and you can test more strategically.

I have a question about onset. There's a lot of times where he starts to go downward at his +1 and +2. There's no food bump unless I give MC at +1, and it's iffy if that works. He nadirs around +3 and is already starting to climb around +4. This is my biggest issue when I'm working, because I'm gone from his preshot to +4.

He eats at preshot, +1, +2, with a little snack at +5 and +8 to help with his tummy acid. He has a history of pancreatitis (lasted almost 3 months), so we give him frequent small meals rather than a couple of larger ones.

I feel like the insulin isn't helping him very much because he's already high by +5/6. Would TR flatten him out where the insulin benefits him longer?
