4/22 - Urgent advice dosing advice needed!

Hello, Manu just tested 148 this morning (yay and ... I don't know what to do!) I'm still new to this and don't understand what I should do. Skip dose? Last time he tested low in the morning, he dropped dangerously low to 47 several hours later. But I have reduced his dose since then. He is on 1-1/5 units Vetsulin, twice/day. I don't understand that entire stall protocol. And he's demanding breakfast. He already had about a tablespoon of Young Again kibble. Here's his spreadsheet. Thank you! If you are around and able to monitor (starting with a +2, will probably also need a +3, maybe +4) I would do 0.5U.

If you won't be around, the safest thing is probably a skip, or you could try something like 0.1U - just barely any insulin drawn up at all, a "hairs width" of insulin in the syringe (but I think you said your eyes aren't too great?)

Thank you! I can be here all day. I will take your advice and post back here to let you know! Yikes. This is nerve-wracking. He was really cuddly in bed for the first time in a while last night. <3

And yes, my eyesight is really bad (and getting worse). But I can use the phone method...

I can't tell you how grateful I am for your advice! Thank you! Thank you! Yes, he seems to be doing a lot better. So, the advice from the vet was to skip the dose. I was still going to give him that hair width dose, but he went into his hiding place in the garage (I call it his man cave :O), and it's getting late, so I'm going to skip the dose. I'll test again +2 and +4. Fingers crossed! I'll post back here, because I probably won't know what to do for his dinnertime shot! <3
 
Yes, you are correct that the guidelines we use for Vetsulin, we reduce the dose by 0.25U any time we get a number under 90, regardless of meter (and definitely not under 68 on a pet meter). I think she's trying to weigh what we do here vs vet advice, plus she said she had trouble with the 0.25U increments.

That said, I would reduce to 1U, and stick with 200 as the no shot threshold.

I'm back and need evening advice! The vet said 150-250 to give half dose (do you think 150-200 is better?), and under 150 to skip. I think Manu is on his way to becoming less insulin-dependent! I gave no shot today. And I just tested him before dinner and it was 193! Woo hoo! But now, what do I do for evening shot? What the vet is saying is give him 1/2 dose. .75 would be difficult for me to gauge exactly. Do you think .5 is a good dose right now? I'll wait until 8:00 pm to give him his shot (it's 7:50 pm now). I know you may not see this in time.
 
I'm a little torn.

I think 0.75U is spot on. Now, 193 is close to 200, so I could also make the argument 1U is fine if you can monitor him tonight.

If you are not confident in the 0.75U measurement or you can't monitor, then yes 0.5U is best.

He's come a long way down I'm glad to see it!

Thank you again so much! But I don't think I gave him enough.:( He was relaxing on the bed, so I went ahead and gave him his shot. I should have waited for your reply. It was closer to .25 units. Here's a photo of the dose I gave him. I kind of blew it I think, but my gut just told me to give him a little less. I hope it doesn't spike too much overnight!

That's almost closer to 0.1U but it's ok. He recovered pretty well with this mornings skipped shot.

Actually, with those syringes - if you align the back side of the plunger (the side touching air, not the insulin) against the bottom of the 1U line that is 0.75U. If you can post a picture of an empty syringe with plunger out of the way I can mark it up and show you what I mean (Facebook would be easier since it's not easy for me to upload pics here from my phone)

Oh, I see what you mean. I will update with that photo in the morning! Gosh, I'm wiped out today. I just tested Manu 3-1/2 hours after his shot, and he tested at 120! That is with the very small dose. Hoping this holds! Thanks again so MUCH!
 
He's on a bit of a mission...I would try 0.5U as your new dose and shoot that if he's above 175. See if that's shootable both cycles. He definitely would have earned a reduction last night on 0.75U.

I fell asleep last night and didn't get a 2nd test in the night. :( But this morning he tested at 189! I'm going to give him .5 units (which would be a 1/2 dose of 1 unit). I am actually supposed to be out for several hours today. It definitely seems like his little pancreas is kicking in! Thanks again for all your guidance. Does that sound reasonable?
