I haven’t been on this forum in so long and I feel bad asking for help out of the blue, my cat who’s been otj for years now has developed signs of kidney disease. He is on hills k/d prescription diet for it. Tonight I got home from work and he was wobbling, fell over when walking and he was hungry as hell. I tested him and he was at 290 and after food was 325 bgl and now after a trip ( and being told 4 or more hours wait) for an emergency vet. It’s at 324 bgl. I know it’s not much to go on but all I can think is Walmart sells Novalin N which he was on years ago. And I can go get that but the pharmacies closed. Does ANY ONE halve any advice? I’m desperate.
 
I’m sorry none has answered you before this. if your kitty has beenOTJ for years, starting to feed hills K/D will most likely knock him out of remission as it is high carb.
Have you tested for ketones on the urine? That is one of the first things I would do. …you will need a bottle of Ketostix from Walmart or a pharmacy. If there are any ketones you need to tell the vet.

Novolon N is not a good insulin for cats. If your kitty needs insulin again you would be much better with Lantus or a bio similar one like Semglee, or Prozonc. These are much better insulin for cats.
Another thing to think about is….cats who have early kidney disease do not need prescription kidney diets
They just need high protein, low carb and low phosphorus diets.
I would not start insulin again until you have established that he is out of remission and the best way to do this is with a fructosamine test.
 
A fructosamine test is a blood test to measure the last 2 or 3 weeks blood glucose and will tell you if Cloud is out of remission.
Looking at your SS it doesn’t look as if you have been testing monthly to check the BG….this is a good idea as you will be able to tell if he is starting to fall out of remission.
FOOD CHART link. Look for low carb food with phosphorus levels of 200mg phosphorus/ 100 kcal. There is a column for the phosphorus in the charts.
Did the ER vet see him?
 
The ER vet saw him, he’s still a bit weak in the back legs but he’s not walking in his whole foot length. He’s at a bg level of 213 right now. Earlier he was 360. He’s gone way down but I’m planning a visit with his normal vet. I had been feeding him a raw diet that was doing WONDERS for his diabetes but the vet said was too high in protein for him. / too high phosphorus? Could I just use a binder? Tiki cat is also a great source here near me. Thank you so much for answering these questions. I’m about to become active on this forum again. It saved his life once.

About half way down is a discussion about diabetic cats with CKD and food. It was suggested the Weruva is a good food for this. I came up with the Family Food version as the best all around. I won't feed to my cat because of the xanthan and guar gums but it is a possibility. You can also add a binder but not as good as a phosphorous restricted food. It depends on if your phosphorus levels are high. Can you post your lab results?
A high protein diet is absolutely fine for early stage kidney disease. A low protein diet is old thinking. The thoughts now are that a low phosphorus level is a much better idea. I fed my early stage crd cat a home made raw diet and added a 1/2 cooked mashed egg white to the food daily. It is low phosphorus and excellent protein.
You can get some supplements now that are lower phosphorus to add to the raw diet.

He has a high neutrophils count which means he most likely had an Infection. How are his teeth?
His BUN and phosphorus are within normal limits and his creatinine is up a bit.
I’m going to tag @Marje and Gracie as she is a whizz with lab results.
 
his teeth I keep as clean as I can. None pulled yet even at his age. He goes for bi-yearly cleanings. I also try to give him crunchy low carb treats or brush them. His vet weeks ago said he has early kidney issue signs. Hence the hills food. Not sure if that’s a normal number for kidney damaged cats.
Update:: he’s being taken in the morning for intravenous fluids to bring down that creatinine and pancreas levels? He will be kept all day possibly over night.
