Glucose in urine but not in blood

Hello All,

This board was a great help for me many years ago when I had a diabetic cat.

I’ve got a strange situation, I am hoping to see if anyone in community has
experienced.

My beloved Ocicat Ducati 10 years old has been having a lot of ravenous hunger. He needs to eat a lot. We went for a vet visit this week with senior bloodwork and urinalysis done.

Here’s situation:

Bloodwork good. No glucose in blood.
Urine had glucose, ketones, and blood.

Ducati has also been peeing a lot. Vet wasnt sure what is going on. We going to redo urine this week with home collection to make sure nothing with values stress related.

Any ideas? Anything to consider or ask?
 
My 2 cents? Sounds like a UTI. Not sure about the glycosuria at this point.

Did your vet suggest a fructosamine test?
 
So I think you mean the glucose range in the blood was normal? There's always some glucose in blood. What was the blood glucose value, and what is listed as the range?

If there's glucose in the urine that just means the blood glucose is higher than renal threshold (in other words, what the kidneys can handle).


How long has this been going on? Like Sue said it could just be a long running infection that's raised BG. The ketones are a bit concerning, what was the value for those?
 
I also suspect some kind of UTI is going on and you should probably have a Culture and Sensitivity run just to rule that in/out.

There's also literature on finding glucose in urine when the blood glucose is normal. From DVM360 "
When Glucosuria in Cats Is a Normal Finding"

Glucosuria usually occurs due to elevated plasma glucose level and/or impaired renal glucose absorption. However, existing literature indicates that nearly all urine samples from humans contain some amount of glucose. Thus, the term “basal glucosuria” is now used to describe excretion of glucose in the urine when blood glucose concentration and renal tubular function are both normal. The suspected cause of basal glucosuria is leakage from distal renal tubules.

In another article, Stress or excitement (cats) can cause glucose in the urine despite a normal BG.

Im going to get lab results on Monday sent over. We are going to do a at home urine collection next week to make sure nothing was stress related.

here’s some other info on what’s happening with him:

his vet noticed he had very bad gingivitis on back teeth. Recommended a dental to remove the molars. Do you think maybe that’s source of infection?

He’s also an asthmatic. He’s had a really bad flare the past month

He is hungry quite a bit. And will sometimes eat my curtains if he’s really hungry.

All bloodwork within normal ranges

I’ve noticed increased urine output.

When I drop off home sample will ask for a culture to rule out UTI. Anything else?

Absolutely could be. Dental disease is one of the top reasons for cats to be hard to regulate.

You can't have a Culture & Sensitivity done from a sample taken at home. The urine needs to be taken directly out of the bladder so it's a sterile sample. Just having urine pass through the urinary tract and out of the body can pick up bacteria so your vet would need to do a cystocentisis (where they put a very thin needle directly into the bladder to get a sterile sample)
 
Thanks! He’s on Flovent inhaler 125mg 2x a day. Lately hasn’t been touching it. Before urine came back we were considering short course of oral steroids but obv don’t want to do that now.

Good to know re: urine. I’ll do the home sample this week and if numbers still same we are likely off to specialist so will ask about the culture then.
