Lots of questions! Sorry in advance!

I took Frankie in to the vet yesterday to r/o a UTI. We are still waiting for the culture but the new thing was that he had really high protein and sediment in his bladder. She said it could be glomerulonephritis (if it persists). We are weaning the pred so she wondered if the prednisolone had been masking or keeping at bay this condition, but I also wondered if the high sugars for weeks (?months) could do this? I think an infection can also cause this so I’m kind of hoping it’s that and treatable, but there weren’t any wbc’s or bacteria seen on the UA.

Anyone have experience with a glomerulonephritis episode? I hope it’s recoverable and doesn’t lead to renal failure…he already has compromised beans. He also has lost 1/2 pound in a month.

I’m frustrated that I had another tiny fur shot last night. It seemed most of it went in but then I could smell it and felt a tiny bit on the fur. Nothing like 2 days ago, which I think pretty much all went to fur. I think it’s the longer needles. The first syringes I started with had shorter ones and I got used to that. Grrr. My question is do I need to start over after a fur shot with 6 more cycles at the current dose before I decide about dosing changes?

Last question: It looks like he’ll be going up to at least 3 units soon. Is it typical to need that much? Or is that on the high side? I really thought his sugars would be lower by now.

A cat needs as much insulin as he needs. Many cats go up to much higher than 3 units and then quickly come down the dosing ladder. Even if you did have a partial fur shot you can take him up to 3.25 tomorrow I think.

Did the vet check his BP?

Many cats go lower at night, so you want to get a before bed test at night. Without tests in the night cycle, you are only seeing half the picture.

A cat needs as much insulin as he needs. Many cats go up to much higher than 3 units and then quickly come down the dosing ladder. Even if you did have a partial fur shot you can take him up to 3.25 tomorrow I think.

Thank you. I posted the next question before your message appeared. Thanks so much for links. Yeah, because of the fur shots, the changing steroids and my vets discomfort with going higher for now (before the pred stops on Sat), I had planned on staying at the 2.75 level until Monday so I didn’t test at night much. I’ll do more before going up. Those nights of sleeping 6-8 hours were amazing though, while they lasted. Lol

With a possible UTI present I would wait until it’s gone to assess kidneys. If no uti I would run the urinalysis again and if still protein is more than +2 run the add on test that checks for proteinuria.

She didn’t check the BP today but he just had a visit to r/o hypertension because his BP was slightly elevated last month. They did 3 readings and all were normal. I’d say that was maybe 3 weeks ago

You can post questions either place. Tanya’s has lots of info on everything cat related. There’s a good section on proteinuria. Protein in the urine can cube and go. It did fur Nax but he never had proteinuria.

Thanks. Yeah, he’s had stage 2 CKD for a while but has never had proteinuria before. I’m praying it’s temporary because of infection. But I was wondering if just having prolonged high sugars could cause it.

Neko had proteinuria, it meant adding a daily med to her regime. She took benazapril which really helped keep her proteinuria at bay. She wasn't having high blood sugars at the time. There are several causes of proteinuria.
