? Steroids and QOL for kitty with cancer

Hey all, I would really appreciate some honest opinions on this.

In November my civie heart kitty Fenton was diagnosed with cancer. Unfortunately the biopsy came back inconclusive to what cancer, as the tumor was too necrotic to get definitive answers, but the assumption was lymphoma, mast cell, or neoplasia. It is unfortunately very aggressive and just a couple weeks ago reappeared in his lymph nodes in the groin. I have opted to not go the chemo route as he's 14, FIV+, and stage 2 kidney disease.

He has been in good health despite everything until a few days ago. His appetite is now poor and his lymph nodes have enlarged to the point where he can't get his back legs to do what he wants.

When we were at the vet just over a week ago, the vet told me the signs to keep an eye on for when he gets closer to the end, and to call him if I feel he's at the point where I'd like to put him on steroids or opioids to make him more comfortable. I am currently waiting on a call back from him about getting him some meds, but I'm still concerned. He literally can't be left alone at this point because he can't get his back legs to work. I don't know if the meds will reduce the swelling in his lymph nodes so he can walk again and have a little more time with us (with him being comfortable). I don't know what to do. I'm not looking to extend his life as long as possible, I just want as many good days out of him that he has left. I don't want to cut him short if he still has a lot of fire in him.

He is bright and alert, still grooming himself, isn't showing any signs of pain, and currently snoozing away soundly. I guess I feel he doesn't seem ready to go yet, but he can't get around at the moment. Does anyone have experience with this? Words of wisdom?
 
Our dog was like this for a year or more at the end, but since he was still happy go lucky and not bothered by much we rolled with it. We just had pee pads all over his area, and we would put a towel under his belly to hold up his back legs to walk around and get some exercise/go outside. He could actually army crawl with the pee pads under him :rolleyes:

How is he about medicating? If he doesn't like it, I personally would not make whatever time he has left full of pills and liquids - especially if he doesn't seem to be in any pain.

I think at this point I'd be looking more palliative than curative, but I don't live with him! I'm not sure anything would do much for lymph nodes really but I don't know

He's very good about medicating so I'm not worried about that, but I can tell he's upset with his lack of mobility. We have mostly hardwood floors and earlier this morning he was trying so hard to get moving he started to cry which broke my heart.

I'm definitely going the palliative route, and since his mobility is bothering him it's something I need to address. I also can't be with him 24/7. I've read that steroids can help shrink lymph nodes so I'm wondering if that will get him mobile again.
 
Prednisolone should help and I hsve used it for several cats near the end and it seems to help. I would ask the vet about pain meds since he really may not be in pain. Buprenorphine, an opioid, is the best but can be a little expensive.
 
Thank you Larry. I didn't speak with the vet but the receptionist quoted from him the walking issue may be more to do with his kidneys than the cancer. Either way he gave me an appetite stimulant and some buprenorphine. I'm glad to hear it's a good choice. I'm keeping prednisolone in in mind too.
 
I don't know if this would help, but when my cat had chylothorax (lymph fluid in pleural space) I have Rutin 500mg TID. They aren't entirely sure how it works, but think it helps stimulate macrophages to get the fluid and lymph moved out. I am not sure if it may help with the nodes themselves, or what it can/cannot be given with, but might be worth a look.

He got his first dose of bupe about 5 hours ago and he just tried to get up and couldn't get his back legs to stay under him and he cried again which has me in tears now. I don't know if I'm just being hasty but if he doesn't improve by tomorrow I think I need to make the call.

Just being objective here - he may need to get used to his "new normal", as any of us do when our bodies don't work the way they used to anymore.

Even so - it will take time for the things you are trying to work. If it were me I'd give him a few days to a week and see, but go with what your intuition is telling you.

Thank you everyone for commenting, I let him cross the rainbow bridge tonight after some more difficulties popped up. I'm hurting too much right now to elaborate but your nuggets of wisdom were greatly appreciated.

Elizabeth, I am so sorry to hear this. It is so hard to make that call, but I think you made the right decision. He knew you loved him and would do what was best for him, and you did.
