? 02/11/22 MICIO: AMPS 186 / NO INSULIN, SURGERY DAY/ 257@ +3 / 469@ +5 / 1u insulin @ 1pm / PMPS 185-

How did the dental go? Any extractions? He on pain meds? At what + time did he get the 1 unit? And lastly (sorry for the questions), how is he doing with eating now?

OK, I see the 1 units was a +7. Depending how he eats, I wouldn't give full dose cause of that 1 unit. Wish they hadn't given it.

@Wendy&Neko just brought Micio home: I tested him at the vet and he was PMPS 185, 1 hour later he's 194.
I'll feed him in about 10 minute (pancreatic enzymes take about 20 minutes to activate) and then give him full dose or less?
Tnx
My $0.02-
You actually shot that 1u at +19.
I’m inclined to say wait until AMPS and plan on resuming your 12/12 schedule provided he will eat.
After a dental you have to make sure kitty can and will eat normally before resuming insulin, incase you need to bring numbers up.

He just ate his full meal, no problems (Just gave him a bit less cause may upset his stomach). He has a patch on his leg as a pain medication and it should last 3 more days. How much insulin should I give him then?

Recapping from the previous condo:
UPDATE: Micio had his dental surgery today, going to pick him up now. Teeth may have not been the whole issues with his swelling and bloody discharge, some abnormal tissue was found on his nasal cavity and biopsies was performed (we’ll know in 7-10 days…I’m fearing cancer though )
His last meal was last night at 11:30pm (they wanted me to stop feeding at 10pm), no insulin this morning and of course his numbers went to the roof (469@ 11 am) so they gave him 1u at 1pm (numbers dropped to 383@ +1 ).
As Sandy says, ability and interest in eating is key.

He ate just as normal, very hungry though. I feed him grinded raw meat with supplements and I made sure there were no pieces for him to chew on it, I also have tons of baby food which he loves a lot!

Sounds like a fentanyl patch on his leg. As for feeding, small amounts spaced out are best after anaesthesia, as anaesthesia can mess with the GI system. Glad you gave him less.

I think your choices are either skip, an option Sandy listed, or perhaps a partial dose. Maybe half? Get back on track tomorrow AM.

Yes, fentanyl. Not sure what to do with insulin....I'm surprised that his numbers were "low" when I picked him up...should I do 1u? Would that keep his numbers a bit lower till tomorrow morning?

The depot of a larger dose will have helped to keep his numbers lower. And some anaesthesia induction drugs (not all) lower the BG's too. Which is why I suggested testing when he got home, then at PMPS to see which way he was trending.

Also, yeah on the negative for IAA!

Great!
Go easy on the food; anesthesia can make some kitties vomit.
I’m in favor of waiting for AMPS to resume insulin. Think of tonight as a fur shot. He will get back on track.
Remember-
“Better a day to high than an hour too low”

Congratulations on the negative IAA results

@Wendy&Neko forgot to answer your question earlier...Dental went well, Dr. did extract 2 little teeth (next to canines on his left side) but most likely not the reason for Micio's facial swelling and bloody discharge draining through his nose, eye and mouth. Dr. found abnormal tissue on his nasal cavity after the extraction of Micio's upper teeth and submitted for biopsy.....unfurtunately I have a real bad feel about that. Micio's a warrior and been through a lot but the idea of cancer is already killing me

The last shot was at +19 (7 hours after after his previous shot). The next shot can be anytime 12 hours after that. So if by any chance you wanted to move his shot time earlier in the morning than you have been doing, you can do that now, as it'll be over 12 hours after that 1 unit.

Keeping our paws crossed for "just a bad infection". :bighug:

I see...tnx for explaining. One more question: Semglee is the first and only insulin I've been giving Micio since last Oct, I still have 2 more pens but thinking about switching to Lantus next..maybe...Do you think I should? Would Lantus be a better choice for him?

The FDA calls Semglee and Lantus exchangeable insulins. Semglee also has a generic produce just called insulin glargine, but again same thing. And it may be cheaper than Lantus. The only thing to consider, if you find Micio reacts to the shots, is a change to Levemir. Lantus/glargine has an acid base, and some cats notice a sting at higher doses. For me, Neko sometimes walked away during the shots. Levemir has a similar action, though onset and nadir are a couple hours later on average, but no acid base. Some people like Levemir for it's flatter cycles, but Micio is doing pretty well on the Semglee now, so not a reason to change.
