? Weaning pred

Sugarcat Frankie is starting the next phase of his prednisolone wean. He started budesonide today. So for the next 10-14 days he’s going to have budesonide daily and pred every other day.

I’m assuming his sugars are going to be bouncing around going high on the days that pred is given and lower (or same) the days it’s not given. What do people do with testing and insulin in these situations? I doubt he’ll have any hypos but should I test more on the days where he doesn’t get pred? Should I continue giving the 1 unit of insulin and not make any dose changes until the pred is done since it’s different each day?

I almost wish I hadn’t started the insulin yet because the data is going to be so crazy hard to interpret.

I know Katherine is away for a couple of days but maybe she will check in, if not I'm sure when she gets back she will reply back

Thank you. He is such a sweet boy. He doesn’t have any teeth left so sometimes his tongue hangs out. I joke that this is what he thinks of diabetes and IBD! Lol
 
How a cat reacts to pred is a bit ECID (every cat is different). I know @Margaret (and Pearl) does/did pred several days apart. I seem to remember she had a day or part of a day with slightly higher numbers on the pred days. Pearl was quite well regulated, so it was easier to see the differences.

With Frankie's mostly high numbers, I'm glad you are starting insulin. Have you looked at the dosing methods here to decide which dosing method you'd like to follow? It might be handy to mark in the Remarks which are the pred days so we can see if there are any patterns related to giving it. The numbers you see and the dosing method you choose will tell you when and if you need to increase.

Side note, if you do a fur shot, could you put something like FS beside the number in the units column, just makes it easier to spot. The fur shot can interrupt the depot, and hence the timing of increases.

Yes, I’ll definitely add which days are pred days. I’m just now better understanding the difference between the dosing methods and I think I’ll probably start with SLGS for now.
 
Hi Shelley, (thanks for the tag @Diane Tyler's Mom )

I would write pred or bude as shorthand in the spreadsheet at the time you give the doses to see if there is a corresponding bump in numbers. I did forget to give Ruby her dose of pred one morning and her numbers were much lower than normal for that time of day, so I would dose insulin very carefully. SLGS is good to start, give yourself a week to establish some sort of baseline, but you might need to switch to TR to fight the hyperglycemia you might see. I was giving just a drop of insulin to Ruby when she started pred (look at our spreadsheet starting in May 2021), then had to gradually over many months increase her dose every 6-10 cycles until we started seeing lower numbers in the AM and PM cycles. Ruby is still a little tough to regulate but my goal is to keep her under 250 or renal threshold for as much of the time as possible to avoid kidney damage.
