Regulation on Pet meter

I’m using Lantus. My cats name is Sammy and hes 16. Could someone help me do TR or SLGS on a pet meter? I don’t know how to higher or lower the dose based on his numbers.
 
Which dosing method are you following? We normally take the numbers at face value, whether pet or human meter. With the exception that the reduction point for a newly diagnosed kitty following TR becomes 68 on a pet meter vs 50 on a human meter.
 
He’s not newly diagnosed. He’s been a diabetic since 2018 and he was in remission three times already. Lantus keeps putting him into remission oddly. So I started to check his blood sugar at home before giving insulin. What dosing method would you recommend? Is there a guideline like this but for pet meters?

Right now he is in the higher 200s pre food and pre insulin

We use the same document for pet meters, just changing the reduction point for TR as I mentioned above. The dosing method is based on what works for you. With TR you have to be feeding all low carb wet or raw food, and get at least four tests per day, the two preshots test and one other each cycle. We change doses based on how low the dose takes the cat, so need some data mid cycle. Other than that, the choice of dosing method is up to your life style and goals for your cat.

So to clarify I can follow these guidelines even though I am using the pet meter? Right?


  • If the lowest point of the curve is above 150 mg/dl (8.3 mmol/L), increase the dose by 0.5 unit.


  • If the lowest point of the curve is between 90 and 149 mg/dl (5.0 and 8.2 mmol/L), keep the dose the same.


  • If the lowest point of the curve is below 90 mg/dl (5.0 mmol/L), decrease the dose by 0.5 unit.


the vet said that according to the blood work he’s regulated on one unit but I should keep doing a curve every 1-2 weeks correct? Sorry for so many questions I just want to be sure I understood correctly. I’m very thankful for your help.
 
Theoe guidelines are for the SLGS dosing method, with a couple corrections:

Hold the dose for a week, and do a curve.
If the lowest point of the curve or nadirs is above 150 mg/dl, increase the dose by 0.25 units (not 0.5).
If the nadirs, or lowest points are between 90 and 149, hold the dose.
If you see a number below 90, immediately reduce the dose by 0.25 units (so reduce as soon as you see that lower number, and not by 0.5 units).

And yes, a curve every week. In addition, test before every preshot to see that it's safe to give insulin. Spot checks in mid cycle will also help tell you how low he's going on the dose.
 
Hi. Thank you again for your help. I am doing the curves like you said to. His vet did a fructosamine as well and she said it was 406. Today was strange though. He is still on one unit. He was 178 pet meter in the morning so I waited 20 minutes and withheld food and test again then at 8:20am he is 244 so I gave one unit. 3 hours later he is 237 then 6 hours after insulin he is 247. I’m worried. Why would his sugar be going up and not down?
