Sorry for the delay in responding. I thought we discussed a starting dose of 2.5U in your previous condo? Anyway, good luck with the Lantus. :-)
have you decided on which dosing protocol to follow? SLGS or TR? If you are feeding only LC wet and can get enough tests, TR will help you get to a good dose faster - and stay at it!
 
i really do try to get more tests, he just cannot stand it. He is getting worse at testing time. having to swaddle him lately! I tried to get some from his paw, but not enough will come out.
I am doing every 2 hours today.
And ya, we had a few talks about doing the 3U.

Im in Ohio

His +2 is 355

In the beginning my vet moved me up a unit every week or 2 weeks when i brought him in which was way too fast. So slow wasnt really an option. Looked like 4 was too much and Melissa caught that, and 3 was getting him into 100s, but his amps and pmps were very high in the 500s, so I was dosing about 3.1-3.2 on vetsulin before today.
I guess I would call it a combo of the two, go slow from 3U and keep trying to get as much numbers as he allows me to get.

It will take 5-7 days for the depot to build. Some cats just get a strong response the first day or two while others take longer. Are you giving treats after every poke, whether successful or not? I started with one my cat seemed to absolutely love and that’s the only time he got that treat. See how testing gives this week. You don’t need to pick a method until then. As long as you feed only wet or raw you can switch at any time.
 
ya, he gets treats every time, don't think he has put 2 and 2 together yet...it has been 2 weeks since i began doing the treats every single time.
Got him eating mostly FF pates now, he is doing very well with them. He gets only LC food

Vetsulin drops numbers fairly hard and fast in most cats, whereas Lantus is aiming to keep them nice and relatively flat. Lantus' "strength" is keeping low numbers low, not bringing down the high ones. So it will just take some patience and being methodical to get where you're going.

i can tell from these numbers already its a LOT different from vetsulin.
Should I just keep him on 3U for a few days to a week to let the depot to build, no matter what the numbers are?
 
Chances are that given the drop from your AMPS to the +8 reading, you're going to see numbers spike back to a higher range. (We refer to this as a bounce. If a cat's numbers drop low, drop fast, or drop into a range that his body isn't accustomed to, the liver and pancreas panic. They release a stored form of glucose and counterregulatory hormones that cause numbers to spike.) It's at least a part what you were seeing with Vetsulin -- the Vetsulin was also pooping out by around +8. The slope to the +8 is much gentler compared to what you were seeing with Vetsulin. As long as you can get a few PM tests this evening, I think you should be OK with the 3.0u dose. My rationale is that the same dose of Vetsulin wasn't bringing Ares into low numbers.

You do want to hold the dose. However, you need to let the numbers guide you. Depending on whether you decide to follow Tight Regulation or Start Low Go Slow as your dosing method, the point at which you give a dose reduction differs. With TR, you reduce if numbers drop below 50. If you are opting for SLGS, the reduction point is 90.

If you are ever needing a vet, I use a cats only practice and the vet is wonderful. She's OSU trained and supported my managing Gabby's diabetes.

With vetsulin, it was blood, feed, wait 20-30 min shoot
When is the best time to shoot with Lantus?
Meant to ask this earlier, and i read somewhere it doesnt matter with Lantus. But wanted to hear it from you guys, thanks.
 
I'm not sure where you read that? Most of us do the Test Feed Shoot (TFS) sequence pretty close to each other, usually within about 15 minutes or so.

With Vetsulin, because it can drop BG quickly, you would have wanted to be sure that you have food on board when the insulin starts to work, hence waiting 20-30 minutes before shooting. Lantus tends to onset around +2, depends on the cat, and how they respond to insulin and carbs. Since you are new to this insulin, my suggestion - it will take some time (and test data) to see how the insulin affects your cat.

Lantus is a different insulin than Vetsulin, so you'll want to be able to get a better understanding of:
  • Onset - the length of time before insulin reaches the bloodstream & begins lowering blood glucose
  • Peak/Nadir - the lowest point in the cycle
  • Duration - the length of time insulin continues to lower blood glucose

I have been reading and have read and re-read the links in Lantus forum...feels like some of them 6x even.
I dont remember ever reading about food timing. I found that "it doesnt matter" info on some website, since i wasnt able to find it here.

That’s a nice first day. You have time to decide on the method unless he has a bg under 90. With both methods otherwise you need the depot to build for 7 days before deciding whether to increase. How did the testing go? Any easier?

its weird. i will have a 1 test out of 3 that goes swimmingly...then the other two are not good. Sometimes the blood flows into his hair (even with vasoline), sometimes it pools up perfectly. other times i think i have enough blood, but im like a tiny bit too shy (so waste strip and poke again)...
I think i will get it, i just need even more practice, and i get antsy too...when i calm down, things seem to go better. Thinking of maybe shaving his ears.

My plan is to test at +3 and +6 at least tonight FYI

I was just reading through Baby Begins Lantus condo...oh man, that is so crazy!

We wasted a ton of strips with error reads at first. He got all kinds of extra treats during the first while. It gets easier and they get more accustomed to it and so do you.
