I am unable to get his blood sugar regulated even with glargine. He also has asthma. Can you please explain how a higher insulin dose can work in reverse?
 
Welcome. We can help you. Are you testing his glucose at home with a human or pet meter? What are you feeding him? How old is he? What’s his name? Any other health issues? What’s his current dose?


We will need you to set up a spreadsheet and add a signature with that information. We are very data oriented here.

I don't know how to do spreadsheet.
He is about 14 as I have him about 12 years and he was not a kitten. He has asthma and nasal congestion. The vet has done fructosamine tests and they come up very high. Began in May with glargine 2units. Did spot sugars at vet. First was 285 and his dose was increased gradually and slowly until 5u BID. Switched to vetsulin for about 6 weeks. I forgot how many units. Fructosamine high. I restarted glargine because I didn't know that I should only feed him BID with insulin. He is on very high protein diet including Tiki cat. I started with 2units twice a day again. I bought the alpha track reader and glucose readings are high. He also had high pancreatic enzymes a month ago do there was pancreatitis as well. I am on 3 units in the am and 4 at night. Fructosamine still high. It was done last Friday. He is always hungry and hardly ever sleeps.
I use the steroid inhaler for asthma but now only once a day. The t4 was normal and definitely not in higher range.
I do find insulin pen to be difficult to use. Sometimes the needle bends. He gets very upset when I stick his ear and growls at me. He was heavy but lost weight. I wouldvthink that the high protein diet and losing weight might lower his sugar.
I was a pharmacist so I do understand a lot about this.
 
Thank you for this information. We definitely can help you as a lot of your advice from your vet is causing him to not become regulated.

We are very data oriented here. You need to test before every shot to be sure it’s safe to shoot. We use a spreadsheet to keep track of the bg. The link to setting one up is in the Basics sticky that was supplied to you by Bron. Do you need help setting it up? If so @Bandit's Mom can get you started in just a few minutes. She is in India so won’t be online for several hours.

Feeding only twice a day is how other insulin works best but not lantus. Most of us here feed several small meals during the first half of the cycle. Some free feed. Food needs to be picked up two hours before insulin so the bg test is not food influenced. Lantus is dosed based upon how low it takes your cat so a second test each cycle between +4-7 will help figure that out.

We use two different dosing methods here. I will supply the link for you to read about them but it’s also in the basics sticky as are the directions for the signature.
 
Pancreatitis is very common in cats, especially diabetic ones. My cat had chronic pancreatitis fur a few years before becoming diabetic. It can raise the bg when active. Enzymes on the blood panel are not the way to test fur pancreatitis. You need the SpecfPL. It’s only worth running if they are symptomatic at the time it’s run. Cats with pancreatitis do best on several small meals a day instead of two large meals. We have a pancreatitis handbook.
