So I was recently corrected that the 3 under 50 doesn’t apply to long term diabetics and it has to be either a drop under 40 or a week in almost all normal numbers to earn a reduction with TR.

Oh, interesting. OK, guess we wait for a drop under 40. Don't think my bouncy boy will go a full week in normal numbers...

I'm not sure I read the earning reductions info that way...

Earning Reductions and starting OTJ Trials

For long term diabetic cats, and following TR there is still a possibility of earning a reduction if kitty goes under 50 on three separate days, and who have shown they don't hold reductions well.

Yes I see, however it is indeed as I noted. The question is whether one can say that it applies to Oberon (ie whether or not he is holding his reductions)
No. It’s 3 times in the 40’s for cats that don’t hold reductions. For long term diabetics it’s one under 40 or all normal for 7 days. I thought as you interpret this until Wendy corrected me. I wish I could remember which condo where I wrote it but I can’t.

It was here : https://www.felinediabetes.com/FDMB...ps-230-2-251-4-209-7-338.255278/#post-2875388

It is essentially the info page I posted previously. Again, my interpretation is as what is italicized about long term diabetic cats not holding their reductions.

I'm not one to comment anymore but I would never let my cat go below 40. Are you truly comfortable with that, Lisa?
It’s how long terms handle TR. You never got comfortable with low numbers. I didn’t like it but I knew I could bring Max right up with hc. Everyone has to do what they feel best with their own cat. Max hit the 30)s on.y a few times.

I agree. Everyone has to do what they feel comfortable doing for their cat. I just think, and have always thought, that the push to the lower numbers is not always safe nor in the cat's best interest.

I think that's why preference is greens. I was not fan of under 40 either

Lisa, for what it's worth, with the IAA complicating things - if he were.my cat I would stick with what you've been doing. 3 drops between 40-50 on different days. In my opinion, "waiting" for a drop below 40 when it seems IAA is breaking + a depot insulin...I would be uncomfortable with that.

Wow, interesting discussion! Right now I'm not sure the IAA is going anywhere. His dose has been going up and down slowly between about 5 and 6.5 U for the past several months. One thing that means is that I'm sometimes making smaller adjustments (0.25 U) even though I'm over the 5 U threshold for switching to 0.5 U increments. Seems like when I go up too fast I overshoot and we get lower BG numbers and some rapid dose reductions (the "IAA is breaking!" fakeout), then end up coming back up. So at the moment I think my plan is that if he drops under 50 three times, I'll take a 0.25 U reduction. (Especially right now, when he has really only barely gone under 50 twice.) But if he drops below 40, he'll get a 0.5 U reduction. I might adjust that depending on how long he sustains the drop. Skips and BCS are also still in play, of course, depending on the circumstances.

How comfortable I am with low numbers depends on how he gets there. When he's been surfing greens and just dips a little lower, it doesn't bother me that much. It's the swan dives out of the 300s that scare me, and that's the reason we check him so frequently even when he's stuck in the pinks.

Yeah … I have been sucked in by Oberon’s “IAA is breaking” scam many many times! You have an excellent handle on Oberon. I have a feeling you know exactly what you are doing!

The guidelines for long term diabetics are once under 40 or a week in green. However, very often this same cat also doesn't hold reductions well and people use the 3 times between 40 and 49. That isn't to say that the rules for long term diabetics include three times between 40 and 49. That's the rules for a cat that doesn't hold reductions well, long term diabetic or not. This is where you have to know your cat, and know what they need to earn a reduction. IAA definitely is a special situation. Lisa has lots of experience of what works for Oberon, and what doesn't.
