Raymond James has been tested for insulin autoanitbody. Test said he's less than 20. Normal is 71. Waiting on results for insulin like growth factor-1. What does this mean and where do I do more research? As I have mentioned before, doesn't matter what I feed him, when I feed him, when to test, what insulin, more units, no improvement. He's no lower than 5 units, as you can see... Thought It was me, but after 10 months, it's not me.. Thanks...

It just means he does not have IAA. But at that dose, he likely does have acromegaly, the IGF-1 will tell. Sometimes there are just high dose cats.

Normal is less than 20%. What is the 71? Is that what Raymond James tested? If you have the IAA results, you should get the IGF-1 results Friday or just after. They get sent to the vet, so call them starting Friday. Once you have the whole picture, we can work on a strategy. (which will include night time tests and possibly a switch to Levemir).
Sometimes there are just high dose cats.
Not at 22 units, all cats that high in dose have some other secondary endocrine condition.
 
What would you say the rough cutoff seems to be?
No particular number. We have kitties with IAA and or acromegaly on as low as 2-4 units, we've also seen kitties not tested for either condition on higher doses. There is also Cushings as another endocrine condition. Plus other conditions that can cause higher doses. I've also seen cats on really high carb dry food on high doses. But, as long as kitty is on all low carb wet or raw, does not have any infection issue like bad teeth or pancreatitis, the 6 units we suggest people get testing done has been pretty accurate.

I feel your pain. Been there done that. I can’t tell you how many times I said, “this insulin does NOTHING!” My boy had Acromegaly. Post over on the Acromegaly forum. There’s lots of good information and help there - and studies and discussions.
 
Michigan State University only runs the Acromegaly tests on Wednesdays. So I don’t know when your blood was sent in, but if it was after this week’s cutoff time, it could be next week when you get the results. At least you don’t have to deal with IAA as well. Keep pushing the dose in accordance with the protocols and you will find a dose to help your boy. My boy was helped a lot by Cabergoline.

Do you have a copy of the paperwork/lab results? Are you sure this is the IAA test and not the IGF-1?

Like Wendy said, the IAA test result is a percentage, under 20 is normal. I hope your vet didn't mix up the numbers. I'd ask for a paper or scanned copy of the results.

Thanks, yes blood sample was sent on Thursday.
 
Don't understand what rough cutoff means???
I was asking Wendy what dose is pretty much a guaranteed indicator that a cat has an underlying endocrine issue. She said usually above 6U, but there are cats on lower doses as well.

I will get a copy of the results. First results in the autoantibody..

It's a medication a lot of us with acro cats use. The treatment options for acro are essentially surgery, radiation therapy, cabergoline, or just managing with insulin. Pros and cons to all of them, but I wouldn't go down that rabbit hole until you get the paperwork and IGF-1 results.

Would be nice if your vet was mistaken, and he does have IAA and no acro.
