Lion - Dosing advice needed

Hi everyone!

**Please tell me if I should post this in the Lantus Forum**

I have finally held Lion steady at 1.5 for a full week. As you can see he bounces all over the place with (generally) the evening curve being lower than the daytime curve.

According to SLGS I would hold the dose.

According to TR – I would increase the dose?

I would very much like to get his highs out of the 400s.

My vet has recommended the next step is to try 1.75 in the morning and hold at 1.50 in the evening. Awhile back someone on this board posted their kitty had cycles similar to this and it worked for them.

Overall, I’d just like some input and feedback. I’m very frustrated that his highs often go so high, but in the past when I tried to increase he would start dropping way too low.

Hi Susan, I can't give dosing advice but I tni k you can start posting on the Lantus forum.
You can introduce yourself and say you were posting on the Health .
There are a lot more members there
When you do post on the Lantus forum you would put in your title
Date, then Lion, then AMPS number every day , if you want you can put his PMPS number
You will need to link the previous days link (which would be this one the link is all the way up top ) every day so members can go back and look at your previous posts

I see you use the Libre, ,and are using g the SLGS dosing method
You definitely test enough if you wanted to switch to the TR method.
I'm sure you read the dosing methods, You could probably get him in better numbers

Reducing the dose: Lantus

The TR Protocol is an aggressive method in itself. The modified version of the protocol is slightly more aggressive. Let's keep all our kitties in the Lantus, Basaglar, & Levemir ISG safe by taking reductions when appropriate.
  • If kitty drops below 40 (long term diabetic) or 50 (newly diagnosed diabetic) reduce the dose by 0.25 unit. If kitty has a history of not holding reductions well or if reductions are close together... sneak the dose down by shaving the dose rather than reducing by a full quarter unit.
  • Alternatively, attempt a reduction when the cat regularly has its lowest BGs in the normal range of a non-diabetic healthy cat (50 - 80 mg/dL) while staying under 100 mg/dl overall for at least one week.

As per the lantus dosing method I copied it for you
Increasing the dose:
  • Hold the dose for 3 - 5 days (6 - 10 consecutive cycles) if nadirs are less than 200 before increasing the dose by 0.25 unit.
    • if your cat is new to numbers under 200, it is recommended to hold the dose for at least 8-10 cycles before increasing.
    • when your cat starts to see nadirs under 100, hold the dose for at least 10 cycles before increasing.
  • Increase by 0.25 unit after 3 days (6 consecutive cycles) if nadirs are greater than 200, but less than 300.
  • Increase by 0.5 unit after 3 days (6 consecutive cycles) if nadirs are greater than 300.

I would definitely ask when it comes to whether you should increase by 0.5 units
If you do decide to post in the lantus forum and you gave a question no matter what it ihete us an example
start off with a ? Date , Lion, AMPS # then Need Advice On Dose

Okay, so it would be increase the dose. I guess I am just scared to do that both morning and evening.

You should give the same dose morning and evening when on Lantus
