Looks like the Tresaderm is significantly raising his BGs (as well as the infection I presume). Last night he was feeling great. BG came down to 239 at +4. So I don't know if he bounced last night or if the Tresaderm has anything to do with it because it does contain a steroid. Not sure if I should go up on his insulin dose or not because I don't know if he dropped below 200 overnight. Really not sure what to do @Wendy&Neko, what's your opinion? I've called the vet to let him know that I think the drops are causing his sugars to rise and waiting for a call back. Poor Max. :( Thanks. Unfortunately most ear meds have steroids in them. Even if the vet recommends something else, it will most likely contain a steroid. I've had good luck getting ear infections to clear up with the Zymox Otic I mentioned that doesn't contain a steroid. https://www.1800petmeds.com/Zymox+Otic+Enzymatic+Solution+Hydrocortisone+Free-prod338153.html

Did you give insulin the night of 8/23? The spreadsheet entry for the units is blank. If you ever skip, please put "skip" or "ns" for no shot in the units column, so we know it was a deliberate skip. Otherwise we don't know if it was just a missed data entry. Why does matter? Well, that was Max's sixth cycle at this dose and you would have increased after that. If he had that sixth dose of insulin. I suspect he did get insulin because he was lower the next morning. Your first yellow preshot!! And he probably went even lower that night when there was no before bed test, showing the importance of before bed tests every night. Cats do tend to go lower at night. Sometimes cats will go high before going low, so don't let those high preshots discourage you from a mid cycle test.

Regardless, this still isn't the right dose and he's overdue for an increase. He should get 9 units tonight. Maybe the increase will help combat those higher numbers too.

Yes I gave it to him. I will fix that. It’s been crazy around here lately. I’ve never skipped a dose for him but had a couple of fur shots which I listed. The vet changed him to Baytril Otic drops which are supposed to be steroid free. I might increase his insulin to 9 units tomorrow am instead if that’s okay so I can monitor him better. I’m always scared something freaky will happen and he will drop too low while I’m asleep. I usually test before bed too. I may have left that blank too but I will go back to my notebook and check that. Thanks @Wendy&Neko

Max is taking more than a couple cycles to resolve his bounce, so he'll likely be safe tonight. Combined with that his depot is still recovering from the partial fur shots to keep him higher. Second thing, Max hasn't shown a strong reaction to a first dose of an increase to date. Some cats do that, and Max might in the future, but overall his numbers are high enough that he's still be safe if you increased tonight.

I'm saying all this because it's very common for people to think increases during the day are the best time to increase. Over time you'll find out at which point after an increase you'll see the most action. For some cats, it's not until the 4th through 6th cycle. Once you know better what cycle Max is more likely to show some action, you'll be able to decide if day or night is better for increases.

So happy your vet prescribed the Baytril Otic drops! I like the ingredients of those drops which are just enrofloxacin (Baytril antibiotic) and silver sulfadiazine which is a very good antibacterial. It is also very healing. I use silver sulfadiazine gel for skin wounds and burns at home - very good stuff! I wish the vet had thought of it before though so you wouldn’t have had to pay for two meds! Good luck with Max’s increase.

@Suzanne & Darcy I know, I wish he had thought of that too. I questioned it and he said topical steroids usually don’t affect blood sugar that much, but I wasn’t sold on that and I turned out to be right after all.
