No matter how much insulin I give, the more his BG increases. Very frustrating. Low carb wet food, three different insulins. Up to 8.5 units of basaglar twice daily. Can't afford testing to rule out why he is insulin resistant. Any suggestions? Keytones, 1.0. Thanks. May as well not increase units, not doing anything, like shooting water.
Nadir fluctuates.
 
The only thing you can do is keep increasing his insulin dose. With nadirs over 300, you can increase the dose every 3 days. For doses between 5U and 10U, increases are in multiples of 0.5U. Above 10U you can increase in multiples of 1U. Let me tag @Wendy&Neko who is our expert on Acromegaly and IAA.

You also have the option of trying Levemir. At high doses, the acidic base of Lantus can sting.

A cat needs however much insulin they need. I would be a bit more comfy suggesting dosing with more night t8me tests, such as before bed as many cats go lower at night.

Hi, thanks for the info. Isn't levemir the same as lantus, basaglar? + 6, today, 369. Never out of the 300's. Has to stay on pumpkin and fortiflora, if not, all over butt and legs. My shaver won't cut his long hair. Have taken PMPS, +2, +3, same, high.. UGG... Spinning my wheels..!!!!

No, it's a depot insulin like Glargine (Lantus, Basaglar, Semglee) but it's a totally different type of insulin.

Neko passed away back in 2016 if I remember right. Some of our members put the GA (Gone Ahead or Guardian Angel) right away, some do it later and some never do. It's a personal decision.

OK, I'm back. First suggestion - get some night time tests after PMPS. Maybe some +3 or +4 if that's around bed time? Many cats go lower at night. We determine how to change the dose based on how low it's taking the cat. Hence the importance of those night time spot checks. He could be going lower at night and bouncing. Of course, it could be bouncing from even yellows if he's not used to them, but we don't know that without the data. The night of 7/24 is probably one such lower drop as he bounced the next day.

I'd also mix up those mid cycle day time tests. Although +6 is the average nadir, many cats don't nadir then or vary from cycle to cycle. Maybe that's your only time you can test due to work, but if not, and on the weekends, I'd try mixing up the tests in the +4 to +8 range.

Next, at this size of dose, a 0.25 unit increase is barely noticeable. I'd move to 0.5 unit changes now. And if he's not seeing any greens by 10 units, you'd move to 1.0 unit increases. If there is some sort of condition causing insulin resistance, you can't sit on a dose any longer than necessary. I'd go to 9.0 units tomorrow.

Thanks. Have taken BG at +2 and +3, PMPS, same, high. This guy ain't going lower at night. Will mix afternoon testing. Last four tests have been in the 300's which is good for Raymond James. Will increase to 9 units. Thanks again. Hope mom is well!!!

She's recovering fine from a local surgery. It's just an ordeal because she lives on an island with no special medical services and I have to go get her, bring her to town and deliver her back. Lots of ferry trips.

You're a good daughter, I would do the same!! I did everything for my mom. I miss her...

Oh my goodness. I was just look er my old posts last night and I said the exact same thing! When you can I would recommend the testing, but I do understand financial constraints.
