Hello! Dallas has been on Lantus for 3 months, but was on a sliding scale for most of that time until I recently found out that this is a big no no with Lantus. I changed his dose to 1.5 yesterday and going to keep it at that until it's time to make a change. When I checked his BG at +8 is was 3.9 and I am assuming he is going to have a number below 11 at his PMPS and I have never shot under 11. Just in case this happens, how much insulin should he get at a lower number? Still 1.5? Please let me know if you can't access his Spreadsheet. This is a new Spreadsheet. I do have an older one from when he was on the sliding scale if you'd like to see that one as well. Thank you!

I can see your spreadsheet on my laptop, but it was created on a FB site by taking ours and modifying it. Unfortunately, those modifications cause it to sometimes blow up on a tablet. And the US tab doesn't seem to be translating the World numbers. Since the server here is in the US, and a large number of the members who help with dosing are in the US, we use their numbers by convention. In other words, most don't know that 3.5 is getting into the lower range of greens, though still safe. As I am in Canada, I do know that. I am tagging @Bandit's Mom , Bhooma can help you get the spreadsheet sorted out so it's usable here. It's also useful to us to put the previous couple weeks of sliding scale dosing in, so we can see how Dallas responded to it.

We do have a couple dosing methods we use here, described here: Dosing Methods: Start Low, Go Slow (SLGS) & Tight Regulation (TR)
Most people pick one or the other and follow it. I think for now, you should lower his dose to 1.25 units. The SLGS dosing method, should you decide to follow it, suggests reducing the dose if you go under 90 (5.0).
We don't dose based on preshot number, but rather on the nadirs or how low the dose takes your kitty. And it's easier to start with that lower dose and work your way up when you have the data to do so, regardless of dosing method you pick.

Thank you for your quick reply. Where would I find the spreadsheet on this site? I'll lower his dose to 1.25 and this is a safe amount to give anytime he's above 5?

Well after reading the link you attached for a second time I see now that if below 8.3 don't give any insulin and if between 8.3-11.1 give a token dose of 10-25%.

This is safest in the beginning until you have more data numbers and you see how he reacts to insulin. If the preshot is lower than expected, you can stall without feeding for an hour and see if it's going up. If it is, it's often enough to go ahead and shoot. If you are ever not sure you can post here for advice. :D

It was 6.6 and decided not to shoot :)

That's happened a few times recently where the preshot is quite low to shoot. Seeing as how you went down to 63, you might consider doing 1.25 to see if your are them able to get 2 shootable numbers. I'm not really a lantus person though, so what do you think @Wendy&Neko

I suggested going to 1.25 units in my previous post. It is best to lower the dose so you can get to a dose you can shoot twice a day.
