I don't understand how the R works with so many kitties on this site. I guess it is some kind of "fast acting" insulin that you add to the existing insulin. Is it only for kitties with other medical issues? I sure hope Oberon comes down for you, Lisa.

R is Humulin R, or "regular" insulin. It's pretty much identical to the insulin that humans produce. It's short-acting, no depot. For cats with "high dose" conditions like IAA or acro, it's a way of giving more insulin without having a huge depot that could be problematic if the cat's insulin needs suddenly decrease, and it can sort of boost the effect of Lantus by giving Lantus a lower starting point. It's also a good tool for cats with a history of ketones. But it's also pretty powerful and has to be used with caution. My target is to give Oberon enough R to bring his BG down about 100 points at about +4 (R nadir). But the R dose needed for that constantly shifts just like his Lantus dose, so it has to be adjusted periodically. A few months ago he needed 2.5 U to bring BG down that much. Right now he only needs 0.5 U to have the same effect. And I only give it if BG is above 300 to being with, so I have plenty of buffer just in case.

Not that it's helping all that much right now... Oberon's stuck in the high numbers again. But eventually we'll hit a breakthrough Lantus dose and I'll simultaneously reduce his Lantus and stop using R as he dives back down the dosing scale, like we did in May.
