? 6/18 What to do about morning insulin with pre-shot test of 110?

Armani had a pre-shot number of 110 for the first time, so I withheld his insulin and he ate breakfast. I will keep testing, but can he have his insulin if his numbers go up (and to what?), and if so, how much of his full dose?

(Note: He has acromegaly and so has a very high dose. We are finally starting to see numbers in the 200s for his nadir. I'm following Tight Regulation so his dose has been going up every 6 cycles.)

Can you monitor him if you shoot? if yes, I think it should be safe to shoot. Let me see if Wendy is available to advise.

A couple things. First, I am glad you shot. You could have actually shot at the 110 if home to monitor. Though to make you more comfortable you could have waited 15 minutes or so, without feeding, then retested to see if he was rising without influence of food. We don’t shoot low numbers any differently if cat has acromegaly if following TR. I shot as low as 51. However, it helps to have data before shooting lower numbers. So get at least a +2 today to build that data.

Second, a clue that Armani was on his way up at preshot, was the low blue preshot, after a cycle where he went from PMPS 255 to +2 of 180. When you see a big drop at +2, it’s a good idea to get a later test that cycle. I would bet he saw green sometime last night, which means likely bounce and higher numbers later today. I would hold this dose 10 cycles, to see if you can spot another lower numbers. and next time you see a lower +2, set the alarm for a test later in the cycle.

Shooting lower numbers is scary at first, but both Lantus and Levemir are great at keeping lower numbers flat. I found their cycles much more predictable. We consider a low preshot a gift. Of course, the first few times I shot blues, then greens, Neko bounced right away. :rolleyes:

Finally, this is cause for celebration, looks like you are finally seeing some better numbers. :cool:

I am definitely celebrating we are starting to get better numbers, and figure I may have some questions until I get used to this new stage (hopefully the good numbers will continue!). I'll hold this dose for 10 cycles as you suggest, and try to get a test during the night if his +2 is lower. And go ahead and shoot at his usual time unless the number is quite low, as long as I'm at home to monitor. Thank you!!
