PZI frustration: Help!

So seriously contemplating switching over to Lantus. It seems he was better without insulin. Slowly have upped the insulin but numbers not improving. Today in the 400s after some dehydrated chicken treats bc he is constantly hungry. Thoughts from the group?
 
I just think it's a patience thing lol Lantus isn't as good at bringing down the high numbers, so really it's up to you. If I had to do it over again, I'd probably have stuck with ProZinc until I got Mr Kitty into low yellows and blues, then made the switch. But he also seemed like he just did not feel good on ProZinc
 
It just looks to me like Lucky needs more insulin :cat: Some cats will get up to a fairly high dose, and then start moving back down. Some just need a higher dose. Of course there would be nothing wrong with switching to lantus either if you would like to :cat:

Yes but I guess my frustration is that I just increased it and it seems to Peter out quicker than the lantus. The poor cat is hungry and although I do give him snacks- which for him is 3/4 to a whole can each time, I seem to be going backwards with the insulin. :(

Prozinc does tend to have a shorter duration than Lantus - it usually lasts about 10-12 hours in cats. Someone with a more trained eye than myself may be able to give you an explanation as to why his numbers seem to be going up like they are. Some of the higher numbers could be a food bump, especially if he’s getting that much as a snack
 
I suspect it's just glucose toxicity. You're doing the right thing with MPM, should get you to break through it as quickly as possible.

Looks like youre getting about 12 hrs out of it, nothing wrong with that!

Are you letting him eat as much as he wants? Is he at his ideal weight? Under/over?

Hi! Thanks for chiming in. He is a big boy. Was 26 pounds, now down to 19.6. He has a waist now and looks pretty good compare to 6 months ago. He loved his dry and that is how he got slowly overweight. Ugh! Anyway, when you say, eat what he wants, I don’t free feed the moist. I usually give him a total of 4-4 1/2 cans of FF pâté per day. He would eat more than a can of FF pâté at a time if I let him but I’m not sure the insulin would regulate it. He’s not as his correct dose so he’s always hungry so it’s hard to judge if he should be fed more. For dinner tonight, I gave him a whole can of friskies pâté. He was walking around checking out my other kitty’s bowls. We will see what his 3 hour BG is at. I guess I’m second guessing whether or not to remain with the PZI or switch to lantus. If I have the chance to revert him back to being normal, I’d like to do this sooner than later. I really don’t want to miss the opportunity if possible.

Gotcha. If you think he's at his ideal weight just keep an eye on him; unregulated cats don't process food well, and can easily get underweight if not careful. You want him to eat what he needs and adjust the insulin around that, not the other way around. When my cat is in good numbers he eats about half as.much as he does when he's in 200s-300s.

FWIW, I don't think switching insulins will change much at this point. You'll end up doing the same thing with Lantus, and honestly it may take longer to get him into good numbers because high numbers aren't Lantus' strength. Totally up to you, I don't see any harm either way.

Ok. I’ll give it a little longer with the PZI. I do check his weight quite often and it’s been the same now for a while. I guess I’ll try giving him a bit more and see how he responds. I guess I have to remember to keep my patient pants on! Thanks for responding. Much appreciated. :)

My only other thought is to not increase by "fattening" the dose. I'd keep to 0.25u increased until you're seeing numbers where you want them to be. The micro-dose adjustments are meant to be use when you're fine tuning a dose. Lucky isn't there yet.

Yes I was waiting for the 1/2 unit syringes to arrive so I was eye balling it hence the fat dose. New syringes came in so we are moving on with those from now on! Thanks for your input! I think the 400 reading was from the dehydrated chicken treats. I’ve seen them spike his numbers before for some reason and sometimes they don’t. I was hesitant to feed him actual pâté as it seems he creeps up at the end of the cycle so I was trying to play it safe. That backfired! He came down at the PMPS and the +3. Taking one more reading now before bed. He has to be kept separate from the other 5 bc he picks on a few others and one picks on him. :blackeye::banghead::blackeye: My life at the moment. Chaos!!
