4/30 Bear stuck in the 300’s - time to increase?

Hi all, my sugar bear has been on 2 units LANTUS 2x per day for 11 days. He was diagnosed the end of February and started at .5 units ( my mistake there, should have been 1 unit) . Then went to 1 full unit then 1.5 and then 2 units 11 days ago. He has a very hard time staying out of the pink. Is it time to increase to 2.5 units? He is on a low carb wet food (Fancy Feast) diet. He eats twice per day at shot time 6:30 am and 6:30 pm. Any advice is welcome. Thank you!
 
I wouldn't say it was a mistake to start at .5 units - but it was held for too long.

What dosing method are you following? TR or SLGS? That will make a difference on how often you increase - and as it looks right now, there haven't been any what you'd call great numbers as yet - so yes, it's time to increase by .25. So tomorrow go to 2.25; if you're following TR, you can increase again in 3 days (6 cycles) if you don't see any low blues or greens.

I would not recommend increasing by .5 units until you get to know how the insulin is impacting his blood glucose with regular increases.

Why doesn't he get food between a.m. and p.m. shots? Does he have a weight problem?

That isn't enough food for a kitty. He should have more small meals than two feedings a day (sounds like vet advice to me??)...at any rate, frequent feedings helps to regulate blood sugars and I cannot imagine that the poor thing isn't hungry between those 12 hours??? It's completely fine to give a little snack several times a day - some people divide the small cans of Fancy Feast into 4 small meals...Luci gets half a can in the a.m. and p.m. and then divided amounts until the first can is gone...

Just something to think about - if you're dealing with an overweight cat you have to be much more careful on how much food they get...but I don't believe in starving a diabetic cat.
 
I agree with Sue. Many of us find spreading the food out with 2-3 mini meals each cycle levels them out. Feeding two times a day worked for the older insulins. I fed at amps/pmps and +2 and +4-5. If at work or needing to sleep auto feeders work well. Are you following one of our dosing methods? I would increase by .25 since you are seeing some blue nadirs. especially if following SLGS.

Elise, thank you for the reply and advice. I will increase to 2.25, fingers crossed that it helps get him into the yellow zone more constantly. I think I am following SLGS. I feel overwhelmed never knowing if I’m giving too much or not enough, is he insulin resistant or we just haven’t hit the right amount for him yet etc. thank you!

Hi Sue, thank you for the reply, I will increase and hopefully we can find the right amount for him. I will also try spreading out his meals more. The 2x a day was suggested to me and it seemed like a good idea but.... maybe not for him. He is not overweight, he is 10lbs. Is insulin something that is prescribed per body weight as well?

Often times it is - by the vets who initially see these kitties - but then after a bit - weight and everything else seems to get tossed out the window as time passes and your home testing determine what your cat needs in the way of insulin and food for that matter. If you're feeding a low carb wet food typically that's a much better food than any kind of dry food.

The issue is that when kitty is not getting the right amount of insulin, they can have a hard time absorbing the nutrients from the food their eating - and many times will lose weight - if not treated a cat can literally starve to death right in front of your eyes - all the while eating everything in sight. So it's important to get to the right dose - of course - that's what we're all trying to do - and then offer multiple small meals to support that level of nutrition that is needed just to get through the day and night too - to feel satisfied and also to help regulate those blood sugars.

Imagine in a human diabetic - being told to only eat twice a day? How miserable a life would that be - and often human diabetics are given the same advice - multiple small meals of high quality foods - a nice balanced meal - but sometimes human diabetics go too long without eating and require a candy or juice to get that roller coaster blood sugar back up to a suitable level - otherwise they'll pass out and then there are other issues...

So it's really best to feed multiple small meals all throughout the day - divide up the can into portions and then let's hope to see that once you find just the right dose those numbers will look better soon. But don't worry - Luci and I have been looking for that perfect combination for over three years - sometimes she has good days and sometimes not so good...

These are the early days for Bear so you've got an excellent shot at remission - just gotta keep increasing on a regular basis (follow your dosing method) and you'll soon start to see results :)
