04/05 Mowgli PMPS = 262 Frustrating Vet Visit

So today I had a vet appointment to have Mowgli tested for Acromegaly and IAA. While I was there I figured I might as well add on a SpecfPL for Pancreatitis, a ProBNP for heart health in the event that he came back positive for Acro, and a comprehensive senior panel with urinalysis to check his Cobalamine & Folate as well as kidney readings.

So the vet arrived and we had a discussion about how Mowgli is doing since she last saw him, which is largely well. However, soon after she asked to verify her notes, especially the part about Mowgli's current insulin dose. When she prescribed the Lantus she told us to administer 1u yet I have upped his dose now to 13u without consulting her... Needless to say, she was flabbergasted! I tried to reassure her that I upped his dose methodically while testing his BG often. I discussed that part of the reason why I wanted to run more tests was due to his high dose requirements, and she seemed to understand. The IGF-1 and IAA test were new to the vet but she was open to sending the samples out to Michigan State for testing. She had never run the ProBNP test but thought it was a good idea to run it. Same with the SpecfPL.

She then brought up the idea of me visiting a feline specialist, to which I responded that I wanted to try to make it work with her instead of trying to find a new vet considering that she is the closest vet to us. And even though she had previously told me that she didn't have that much experience dealing with diabetic cats, I reassured her that as long as she was open to learning alongside of me about these possible underlying conditions, then I would be a happy client. She then proceeded to say that she needed to call her boss to discuss how much it would cost to run all these tests (she hinted that it could be a lot).

After a long wait she came back and told me that unfortunately they could not help me...Her boss was uncomfortable with how much insulin we are giving Mowgli, saying that they have never seen a cat need this much insulin...Furthermore, he did not like the idea of having to work with MSU to get the acro and IAA tests done. I tried to tell her that there's nothing to do apart from mailing the samples with a filled out form but to no avail. I even asked if they could take the blood sample and give it to me so I could mail it but she said absolutely not. She apologized for not being able to help me and urged me to be careful with the insulin dose. She even said that we don't need to keep increasing his dose since our average BG is around 300. She said that's pretty good!

Anyway, with all the stress that Mowgli went through with the car ride and barking dogs I decided we'd reward him by not testing him much today. The end and back to square one.
 
Ugh, sorry about the vet visit. My vet didn't know how to do the tests either. So a member with an acro near me told me about a local lab that did all the dealing with MSU. Basically I told my vet which test numbers to order at the local lab and she sent it there and they sent it to MSU. Probably cost a bit more for me, but it got done. Someone here said they've used Idexx to do that, I just found that test 3237 is the IAA one and 3241 is the IGF-1 test. Check out this page. If your vet deals with Idexx, maybe sending the blood to Idexx to send to MSU will work.
 
Darn it. I should have checked the Idexx directory. Had I known I may have been able to talk them into running the tests.

As it stands, I think the head veterinarian was mostly uncomfortable with the fact that Mowgli is on 13u and just used the fact that they've never worked with MSU as an excuse to not work with me. At the time I felt it was okay to divulge the insulin dose in order to justify running the tests for IGF-1 and IAA, but who knows.

I'll have to think about whether it's worth calling back with this new information haha. How does one convince a veterinarian to take me back as a client? Is it similar to an ex-girlfriend?
 
Oh I am so sorry to hear it. I would have been screaming the place down (which wouldn't have helped....) in frustration!

Does the vet practice has multiple vets? If so you could try a different one and say you just gave 7U of insulin? My experience is that in my cliinic that have multiplevets they never read the whole history...

Any othe rpractices not too far? Perhaps you could contact Idexx or MSU, and ask who in the area have done the test?

I have begun to look for other vets and have found one that is willing to run all the tests but she doesn't have an opening until late April. I will be reserving that appointment in the event that I can't find any other vets.

BTW, funny that you say you would be screaming the place down. When I told my mom what happened she said she would have started to make a scene too haha. I will admit that I was close to telling the vet that the reason why she has never seen a cat that needs 13u of insulin is because she obviously hasn't treated many diabetic cats. But in the end I bit my tongue and left.
 
Wow...that IS frustrating! I'm so sorry to hear that story. The thing I don't understand is why they won't just run the tests. We are the ones paying for them! If they then don't feel comfortable treating the cat, then they can "boot" him, to use the term my vet uses. (she is great...when they say "boot", they just mean to a specialist) But I never see the harm in having data to work with.

That's exactly what I did finding a new internist for Minnie back at the end of Feb - they didn't have an appointment til end of April so I took it and asked to go on their waitlist. Luckily they had a cancellation and we got in at the end of March! Definitely worth asking about a waitlist. Good luck.
