At the vet Friday night my cat was diagnosed and her number was 16.45 and prescribed 2 units, twice a day of Lantus. I did home testing today and her numbers came back 11.6 and 13.1 and she hasn't had insulin in 22 hours due to getting use to scheduling.
I'm concerned about the dosage now.

** Yes, she's only had 2 units Friday night and 2 units Saturday night so far.

Can you give some additional details regarding diagnosis? Were there symptoms of diabetes, like excessive peeing and drinking a lot of water? Other than the BG test, did the vet run any other tests like a fructosamine, or urinalysis? 16.45 mmol/L is 296 mg/dL, so that's higher than normal, but some cats can have stress hyperglycaemia (elevated BG level as a result of the stress of visiting the vet), so having a few other tests to verify like fructosamine and urinalysis would make sense. If there was a urinalysis, it should indicate whether glucose was present in the urine, and as well, please check whether there were any ketones present.

In any case, those tests you took, when were they in terms of hours after insulin?

Also, what are you feeding? Any dry food in the mix? We have found that dosing twice a day is best, since cats metabolize insulin differently than humans, so I would say going forward, it would be better to give 1u twice a day, instead of 2u once a day; however, it would be good if you could clarify some of the questions above. 1u twice a day is generally a good starting dose, although if you recently removed dry food, we would need to know that as well.

ETA: and please let us know whether you are using a human meter or a pet meter.

So you shot 2.0 at night and nothing in the morning?
Yes, apparently her vet had instructed her to do 2U once daily, so she did that on Fri night and on Sat night, but that is all. On the sister FB group this evening, Karen Nicholson has been helping her and she has received some general advice about 1U every 12 hours as the normal starting dose.

Several others of us have shared our experiences with our own vets prescribing not-so-great to start with.

I do believe she may have decided to go with the 1U every 12 hours after all the testimonials today, and everyone has been encouraging her to get help here on FDMB. I think the PMPS recently was 13.1.

This is all I know. I will bow out - not trying to interfere here, just complete the picture a bit!

There were signs , drinking alot of water, peeing. They did a urine test first and thats where she noticed so then they did a blood test. That's where she had a reading of 16.45 mmol/L. They told me to give her two units of Lantus, twice a day (so far I only did it once in the evening for two nights because I had to go back the the vet in the morning and the next morning I slept in so they told me to skip)

I got a testing kit today. She had two units last night around 830pm. She ate at 10am this morning and was just picking a tiny bit every couple hours. I tested her today at 530pm so around 22 hour after her last dose. She tested the first time at 11.6 and the 2nd time at 13.1. I took her food away, waited two hours and tested her again and it came up 13.1 again.

Since the first night I started mixing her food with fancy feast pate and she loves it, so tonight she had more soft food than hard food.

Thanks for the additional clarification, is your test kit a human meter or a pet meter? We are often a bit more careful when kitties have dry food removed, since we have had more than just a few need less insulin and/or become diet-controlled only by the change to lower carb food. Since there is still some dry in the mix, my suggestion would be that 1u is fine twice a day. 2u twice a day is too high of a starting dose, IMHO.

please let us know if you have any questions, and welcome :)

It's a human tester. And thank you so much. I rather start lower too. I was terrified of given her too much.

You are welcome! There is a lot to process in the beginning, and we've all been where you are. Please don't be afraid to ask any questions you may have, there are many people here with plenty of experience who are happy to help. I have more info for you, but I thought I'd let you absorb a little :)

Thanks, it's all I've been doing since I found out. I'm so tired lol
