3/20 Minnie PMPS 364 (OR 278 on Freestyle), +1 300 Freestyle, +7 317 (282 Freestyle)

It’s not enough juice most likely but you need to wait 6 cycles to see. If you don’t see yellow or blue numbers tonight I would take a .50 increase tomorrow morning.

Come on Minnie, remember how nice it is in the blues and the greens, let’s get there again:cat::cat:

good luck Liz with the increase, we have our paws crossed for you two :bighug::bighug: or maybe with blues tonight, who knows what does Minnie have hidden in her sleeve :)

Sure, will do. What I don't understand and is making me stressed is how her numbers are going the wrong way after an increase, AND staying pretty much flat for 6 cycles. She was down in the blues and greens at 4u. :banghead:

I don’t know if this will help you but Chico did worse in 1.5 compared to 1.25. As well he did well on 1.75 in the beginning but then got much worse on it- he went back to blacks. Try not to get too nervous about these things, all you can do now is try to find the better dose for Minnie, we cna’t always understand why they react in a certain way.

I know it is easy for me to say from a side and that i, just like you, get upset with any bad day Chico has. Taking best care we can of them is the most we can do. You are doing a great job and we all follow Minnie’s progress closely :bighug::bighug:

Can someone help me understand what might be going on here?
How is Minnie acting/behaving compared to a week ago? How has her weight been doing the last couple months? Did you ever find the Freestyle strips? Asking this last one cause numbers seemed to go off the rails when you switched meters so just trying to eliminate that possibility.

A cat's dose requirement can change over time, there are many reasons, from needing a dental, an infection/inflammation, a secondary condition (pancreatitis, hyperthyroidism, heart or kidney disease, ...). The numbers on the spreadsheet won't help us answer any of these questions, but the Minnie report might help.

Funny you mention the strips! I did find them and ordered more - pulled them out after the +7, so I will try them again.

Her behavior doesn't seem too different e.g. she's still playing with the wand (first noticed on 3/11), asking for cuddles and brushing, lots of napping. Only noticeable difference has been increased urination and maybe appetite, and drinking water the other evening though that hasn't continued.

Weight has ranged from 11.3 - 12.3lbs since Jan30 but that's not a trend - I've been doing it with/without her on human scales and calculating the difference. It's the very last column of my s/s after all the remarks. :) Lots of fluctuations day-to-day that didn't really make sense so I probably should get baby scales if I want to track that more accurately.

First consult with the new IM is Wednesday so I will give them all this info as well as asking about the acro/IAA test.

I saw that WSU has a clinical study that covers the IGF-1 test plus imaging for cats with an elevated IGF-1, so I contacted them about enrollment. I'm guessing you might be familiar with the study? Looks like it was posted in Sept 2019. Interested in anything you know (or opinion you have) about this! I'm in Seattle so it's possible (5hr drive if imaging needed) though I'm not sure the benefits outweigh the effort and stress. I will also get quotes for imaging costs from the vet for comparison.

Thanks so much for your response - even if it doesn't explain it, at least I am better equipped to ask questions at the IM appointment. I feel like I'm living in the pink twilight zone.

This is like what Darcy has done in the past. His numbers were always going the wrong way and flat... he has been a pink pancake for a LONG time! Sorry. It is so frustrating, isn't it? Hug!

No, not familiar. I think UC Davis did one in the past too - they have one now on the hypophysectomy surgery. My only concern with the WSU study is the imaging. Although interesting to have imaging done, most times the IGF-1 will tell you enough. And the imaging is not 100% definitive. See this article, page 1093. (2020) Updates in Feline Diabetes Mellitus and Hypersomatrotropism That article also has a list of things that cause insulin resistance. One other thing against imaging is that it means anaesthesia. And that's not something I'd do unless there is good reason. If you plan to go ahead with the surgery, WSU does do it. They used to do SRT, but don't any longer. They don't have a huge track record on the surgery. See post 13 in this thread.

Thank you, that really helps. I will read the links. I already did some preliminary reading of posts in the acro group which is how I ended up finding the WSU study. And also confirmed that I don't want surgery if we get to that decision point.

My plan is an increase of 0.25u to 4.75u in the morning based on the Freestyle numbers which are <300 (even though the s/s color coding is based on the higher Novamax numbers) - assuming no sudden Saturday night drama tonight.
