Advice requested

Hi! I could use some advice.

My 14 yo cat has been diabetic since 4/2017 but I feel like I’m now starting from scratch again. He’s had A Lot going on in his life since mid-Dec, which is when I started struggling to keep him regulated (my other cat/litter mate was gone for a wk & then we had a baby at the very end of Dec, which has been a HUGE adjustment that I know has stressed him out).

Located in the US
Diagnosed 4/2017
Lantus since fall ‘17 - 1U, 2x/day
- I was using solopen, recently started using a syringe w/the solopen
Never has had ketones
Weight: 9.5 - recently 10 pounds (which is good, he was at low end of ideal. Vet rec that he gain a little weight in the beginning of Dec., so I have been feeding him an increased amount of food. He will eat whatever you give him- always has been this way)
Food: Tiki chicken/chicken & eggs and Caru turkey/chicken. I recently switched to EZComplete with cooked chicken.
- If I was not home testing, I would not know that he’s been high as his hair, water consumption, activity, etc has been pretty normal.
- chart attached, I recently started feeding him smaller meals 4x’s per day as I saw rec on here. Before I was only feeding him 2x’s per day.
- I increased his dose to 1.25U (indicated on chart)
- I hate him being high, so sometimes when he’s in the 400’s, I’ll give him an early shot & just monitor him (idk if that’s a good idea or not?)
- no other known issues, last blood work & teeth cleaning end of September (has had 1 tooth pulled)

Any advice is greatly appreciated!

Hi Welcome :bighug:

I can't access your SS - do you know how to edit the share settings to be "anyone with the link"? And then the ideal place for your SS link is in your signature (which you have set up so nicely!). Put the link to your SS on it's own line (see mine as an example) - you can write the text "Felix SS" and then highlight and click the chainlink in the tool bar and then put in your link.

This is not a good idea. I know we all want to fix those high numbers immediately but we need to do it safely. Shooting early can act like a dose-increase.

Hope that helps a little bit - you will also want to browse the posting guidelines - this board gets a lot of traffic and having posting guidelines helps those with experience easily see who needs help.

An early shot will a t like an increase. What’s his ideal weight? Being very hungry can be a sign of both too high or too low a dose. We need to see the spreadsheet to try and get him on track again.

He’s been between 9-11 pounds since diagnosis, around 9.5-10 for the last 3 or so years. My understanding from the vet is he should be around 10 pounds, which is where he’s at now. He’s not begging for food but he’s a cat that if you put food in front of him, he will eat it & gets into anything you leave out (has been like that since he was a kitten).

I only give him a shot earlier than normal when he’s high & I’m here to monitor him (I keep him close by when I do this). Not sure if that’s a good idea or not & would like to get him back on track and possibly even better. Prior to mid Dec, he was usually between 80-300.

Are you shooting lantus 3x today (and 02/11)? @tiffmaxee did you notice this by chance?

I did, the blue line through it is when I started trying to follow the tight regulation protocol. Which one would you suggest following? I only gave an additional shot because he was so high (on both dates), I probably shouldn’t do that.

Do you suggest an increase tomorrow morning to 1.5 or give it another day?

I’m feeding him all wet food. Recently started feeding him EZComplete with cooked chicken. (Never any dry food)

NO! Do not increase. I was saying the 1.25 was needed. I also said we need to decide on a dosing method before changing the dose. I’m going to review your ss again. Be right back.
 
Lantus is never dosed based upon AMPS or PMPS. It is dosed based upon how low it takes your cat. We test before shooting to make sure it is safe to shoot and the bg is high enough to safely shoot knowing the insulin will bring him down. Don’t ever feed two hours or less before amps or PMPS because the bg will be food influenced.

Did you shoot extra insulin today? If so, I would not shoot tonight at all.

We will help you. Vets just don’t explain much. Don’t shoot tonight please and spit in the morning before shooting as well. :bighug:
 
Today I did & will not give another shot until tomorrow AM. The last 3 days I did not & his nadir is 192 based on those dates.

I was only feeding twice a day, AM & PM at the time of the shot (after the shot). However, after reading the link you sent and these forums (+ facebook), it sounds like smaller meals are better? So, I just started feeding him 4 small meals- 1 after/at shot time and then +3 or +6 ideally (I screwed up once & that was the +9 time but I’ll wait if that happens again).
 
He’s underweight right? So feed mini meals as often as he wants. I tried to feed the bulk of the food by +6 he but it’s fine to feed up until 2 hours before amps/PMPS if he’s hungry for now.

Kk sounds good! The Vet puts him in ideal weight range but then says it’d be okay if he gained a little weight. I’ve been told that by 2 different vets.

One last question for you, it says if you get a nadir under 100, you continue for 10 cycles before increasing - but how low is too low to give a shots?
 
If he’s under 100, post and ask for dosing advice. It Josie have shot low 100’s before. Just be sure you have syrup or high carb food, honey or syrup, and plenty of test strips. If in doubt, post for help. Keep asking questions. Just remember no shots before 12 hours, even if high or you think it was a fur shot. Better a few days high than a hypo.
