? 09.02 Chico AMPS 551 Lantus Starting dose

Dear all,

We have finally received our syringes and today we can make the long waited transition from Caninsulin to Lantus. We would like to ask for your opinion on the starting dose for Chico.

Currently we are giving him 1.5 UI caninsulin and his nadirs are pretty, the preshots are very high but that is due to the way caninsulin is working. Therefore we are considering to switch to 1.5 UI of Lantus as well. We would feel comfortable that he won't be going too low too quickly and with the carry over we would hopefully achieve flatter curve.

What do you think about it? Is 1.5 UI correct or should we start from 1.25UI (Chico weights 5kg) and gradually increase if not sufficient?

While we wait, How much does Chico weigh? I think this is a factor in deciding, but again, I am not sure how the conversion works.

Were you hoping to start Lantus this morning? Or...do you have time to decide?
Chico weights 5kg, we still have 5 hours until the evening cycle starts.

I didn’t see this before I popped off but I just came back on so thank you, Tanya.

Marina you have a good sense for this. Chico’s had a few nadirs below 100 so 1.5u is working for him. However, sometimes we see a quick response to Lantus on the first cycle; I would suggest starting him at 1.25u of Lantus. Let’s see how he does but it’s only one dose from getting him back to 1.5u if we need to.

Glad to have you all with us and good luck with the switch. Lantus is a great insulin.

Do I remember you said in a previous thread that you were thinking about doing TR when you switched to Lantus?

I agree with Marje 1.25u is a good place to start, just in case he has a strong reaction to the switch.

We want to start tonight, our shot time is 9PM local time which is in 3 hours.

Thank you and to @Marje and Gracie for the advice. Will we have to wait for the 7 days in order to decide if an increase is needed or will we be able to decide after 3 days? From what we read a decrease is granted if nadir goes below 50 in the next cycle already but i am not as certain about the increase.

Can you please advise as well how to store the Lantus ampules after opening it? In the medicine description it is written that the opened bottles should be stored outside the fridge but on the forum we read that it should be stored in the fridge even after opening :confused::confused:

How long you hold onto the starting dose depends on what happens to his numbers. If for some reason he was pink and red for the first three days you would take him up on the fourth cycle. But you will want to wait at least 5 cycles if you see some movement in his numbers

  • Hold the initial starting dose for 5 - 7 days (10 - 14 consecutive cycles) unless the numbers tell you otherwise. Kitties experiencing high flat curves or prone to ketones may want to increase the starting dose after 3 days (6 consecutive cycles).
  • Each subsequent dose is held for a minimum of 3 days (6 consecutive cycles) unless kitty earns a reduction (See: Reducing the dose...).
  • Adjustments to dose are based on nadirs with only some consideration given to preshot numbers.

In the Fridge, the ampules are stored outside for humans because, they get through the insulin very quickly and the mechanism doesn't work properly when cold. We store it in the fridge because it lasts longer and we can use up every drop.

We are on the same time zone :)

I see Gill has responded. I agree that we should see how the first six cycles go and then decide.

I believe we have 1 hour difference, i started writing the message and then got distracted with other tasks and finished writing later :)))

Geographically, Spain should be on the same time zone as UK and Portugal, we are on the Greenwich meridian, but we are in the same time zone as Central Europe:rolleyes:

It would be good if we could see clearly on the SS when you made the swap to Lantus By having a line run across saying something like Starting Lantus.

have a look at Georges ss 2019 tab you can see the sort of thing I mean where he restarted lantus.
