11/29 Willow- immediate results on Levemir! AMPS 287 / +3 276 / +6 130 / +8 179 / PMPS 542

I was waiting for a Levemir prescription for a few weeks, and keeping Willow at 10 units of Lantus in the meantime. Her numbers weren't low, but she hated the injection and was biting me. Stopping the increase helped with this, and I'm also waiting for cabergoline-- the vet OKed it and I'll get a shipment from Wedgewood.

Started Levemir last night at 8 units, and the effect has been DRAMATIC and IMMEDIATE! I wonder if I should have started at 7. I am not an overnight tester- should I be worried about hypo?
 
With lev the nadir is often at the end of the cycle, even +12. I wonder what the night nadir has been. If worried about dropping I would leave out food. It’s too soon to know when Willow will nadir after just one shot and we don’t know what her nadir will be even today. I’ve only used Lantus. See what Wendy and other lev users think you ought to do. @Wendy&Neko

It looks like the nadir was mid-cycle today (updated +8hr test). I'm worried the effects of the depot haven't happened yet and so I might go down to 7.5 tonight.
 
When people switch from Lantus to Levemir, we suggest doing so at 70% of the Lantus dose. So yes, 7 units. The reason for the reduced dose is that some cat have a strong reaction to the switch, such as you are seeing. However, having said that, the lowest you are seeing is 130, so not dangerously low. If you wanted to back it down to 7.5 units, you probably could.

Ok thanks Wendy! By the way, does this mean what I'm seeing won't last, and her numbers will probably go up again? I was hoping it was a result of the insulin being more effective for her, rather than a shock of switching insulins.

Hard to say at this point. Some cats need less Levemir than Lantus, some need more. Neko's dose didn't change much. But, she did get better duration and much flatter cyles. Which was much easier on me and better for her.

I see. I was hoping for less bouncing, but Willow came up to 542 at PMPS.
 
It takes them a while to learn how to work with the new insulin. That bounce was expected given today's blues.
