Need to understand where we go from here. Dolly started TR on 11/14, increased to 1.75u. She has been on that dose since (15 days). She has dropped below 50 one time, dropped below 70, 5 times (I think). I do not want to decrease her dose, afraid of returning to yellows. Her nadirs have been under 200 every day but 2 since starting TR. Her nadirs have been under 100 - 5 out of the last 7 days. I have read the protocol so many times and I am still not sure if I should continue to hold her current dose or if I am supposed to increase her .25u (scares me). I want her to be heathy and hopefully go into remission again, a healthier remission.

I think that you have room to increase but I know it’s hard for you to balance your life stresses with lower numbers. You are seeing nadirs under 100 but still pinks and yellows. I know a second remission is more difficult to achieve than the first and it’s suggested that you be more aggressive. Being an increase is scary for you which I understand. I think your goal might be better to be getting her under renal threshold and if she achieves a second remission that’s even better. I’m going to tag others to see what they think.

The low numbers don't scare me it's the days when I have to go out and manage her numbers around that. We have been isolated for over two years so we are usually home. It's the doctor's appts and marketing that take me away from home. It's also the sleep deprivation, I am not young and live with chronic pain, my husband has a seriously compromised immune system. The broken sleep is hard for me. I am sure I am not alone, I know everyone has their issues they have to deal with and manage to get their cats healthy. Aside from all of that I still want to move forward with TR, I need direction, don't fully understand the protocol.
 
A couple of definitions from the New to the Group Sticky Note:
  • Well regulated - generally below 200-250 (11.1-13.9) and often near 100 (5.6), no hypoglycemia
  • Tightly regulated - generally below 150 (8.3) and usually in the 60-120 (3.3-6.7) range, no hypoglycemia, still receiving insulin
Those are your goals, ideally tight regulation. Dolly isn't there yet. So needs that increase. Holding too long, longer than the 10 cycles suggested by TR, can allow glucose toxicity to develop. Dolly is seeing lower nadirs, so you absolutely only want to increase when you can monitor closely. Sometimes cats only need a few cycles at a higher dose to get with the regulation program. Or sometimes they just go flatter. Have you ever looked at Tyler's SS? Diane kept increasing, nervously of course, and one day Tyler's body just "got it".

So you have an autofeeder that can help to feed Dolly and keep her safe on those days you have to be away, or to help keep her safe while you are sleeping?

Not only is Dolly a social eater, she won’t eat cold food because she can’t smell it, she just walks away. I look at Tyler's spreadsheet, almost a year of practically round the clock testing, I am not physically capable of doing that. I've gone 48 hours without sleep and it took me a week to recover. I assume Diane found a way to get sleep between tests but it doesn't work for me. As it is now I am exhausted all the time. Dolly and my husband are the center of my universe, since Covid entered the picture, even more so. It's easy to say you would do anything for a human or a pet but the reality is my body (and mind) don't always cooperate. I have looked at other spreadsheets like Tyler's, Chris R and China for one, and others. That hour between tests disappears so quickly, its as if that's what our whole life has become. I also hope to have her off of insulin one day (if possible). The spreadsheets I've reviewed, the constant testing and insulin, if only a drop, never ends. I try not to grab Dolly for tests, I've been trying to get her out from under a table, anyway, she's at +11 and her BG is 67. I am tired and don't know what I am talking about right now. If increasing her is what I need to do to make her healthy, then that's what I'll do. It looks like tonight she's going to be low at her PMPS. This has never happened before at night. I am sorry (sorry for saying sorry) for going on and on. I'm not sure what I want to say. I know I need guidance, and now tonight I will give her insulin but it makes me nervous. I have to stop talking for now. Thank you both for being there.

she got her insulin at 7:00, pacific time. When you wrote me she hadn’t had her insulin yet. I’ve never seen that message from FD before, it really threw me off.
I am always grateful for any and all help I’ve received from all of you. I hope everyone knows that. I know how giving you are of your own time to help people like me give the best life to our precious cats. Thank you so much.

Her usual raise after her PMPS, so disappointing. She is full of more energy than I've seen in awhile, it's a joy to watch. Have a good sleep when you can, thank you. I will raise her to 2 units in the morning. Going to bed, no pattern of drops after her nighttime yellows. :)

Sounds like she is feeling good.
Yes, very good, but she has suddenly decided she’s not going to get poked anymore. Days like yesterday are hard for us both and every now and then I hurt her, makes me feel terrible. She knows if the microwave is going it’s test time. Sometimes I run the garbage disposal to block the noise but even that doesn’t always work. :stop: Have a good night.
