Hi, I'm Elizabeth and my 12 year old tabby mongrel Lily was diagnosed with diabetes this week. She'd been losing weight and getting a bit stiff looking for a while but given her tendency to access my first floor flat via the balcony I assumed it was just age and antics. She's always been tiny so I just thought she was an old lady getting a bit thin around her back end. You can imagine how terrible I feel now.

She pulled a toe pad away from her foot last month, and the vet saw her and fixed her up and said she had a bit of a rubbery intestine (she's always had a sensitige stomach) but otherwise was fine. She was 3kg then. I was worried she was going to be cold staying out overnight as she likes to do, now it's getting colder. I changed her onto wet food to encourage her to eat more and her appetite went through the roof but it really upset her stomach - turns out it had wheat gluten in it which she must be sensitive to. Kept catching her on the kitchen surfaces and the dining table which she doesn't usually do unless there's something really irresistible up there. Three weeks on I was worried she was looking even thinner and when I weighed her she was 2.5kg. She was looking really stiff, moving awkwardly like she was uncomfortable. She also had been very grumpy and not purred for days. Took her to the vet and they did blood tests for thyroid and diabetes and her blood sugar came back at 25mmol. She'd lost all that body fat because she couldn't use the glucose. The vet asked me to get a urine sample to check for ketones. Finally after an 11 hour stand off she did one for me at night and I took it in yesterday morning.

Her legs were really struggling by then. We don't really know if it's neuropathy or weakness. The vet had me bring her in at lunchtime and they kept her in to give her fluids and start her insulin. She responded really well to the insulin, just one unit/mg (?) and they let me take her home at 6pm. She had one tsp of Sheba (I've ordered some Carny and some Boxita) and purred when I put a tiny piece of steak down for her but couldn't eat it. She got to the litter tray twice but she ended up sleeping on the floor near it rather than making the journey repeatedly. I didn't think she'd still be with me by this morning but she was. I took her back this morning and her glucose was at 21.5mmol. The vet taught me how to inject her and sent her home with me. She's still not eaten and is just lying shattered on the sofa behind me with her third eyelid across. I've tried putting the pate food the vet sent me back with on her lip but she's not interested. I'm worried she'll go into a hypo if she doesn't eat something.

A very kind local lady has offered me a brand new glucometer with all the bits to go with it, which I'm going to pick up later. I really want to see Lily eat something. I'm worried this is the end for her, she's so fed up and weak. I feel like a terrible mum to her. If I'd spotted something earlier she would have made a much faster recovery. I'm off my own food now!

Anyway, I'm keen to learn as much as I can and look after her the best I can and was directed here by someone who said it was a really great place to get help and support. Sorry this was so long.

It sounds like you have a bit of an emergency going on here. I believe you need to force feed your poor baby Do you have any baby food the kind without onions and garlic? She may eat that. Has she been drinking at all?
I think the best place for her is at the vet at the moment. Do you have an emergency vet nearby? Insulin and no food is an emergency on its own, with all the other things you describe I'm worried this cant be righted at home.

If you can try anything deli meat, human tuna, what ever her fave treat is... and syringe water into her mouth aim for her the inside of her cheeks so she doesnt aspirate water.
What insulin are you using?

Do you have a gluco meter? I would get down to a drug store and pick one up. Just a regular human gluco meter.We have videos to show you how to test blood sugars. You need the meter the strips to catch the blood and lancets preferable 29 gauge.
 
Hi, thank you for your reply, sorry I disappeared for a bit! She has eaten a little bit of the pate food the vet gave me off my finger so that is promising. She's more alert now. She had the insulin at the vets approx 3 hours ago. I was trying to get her to eat something before I went over to get a glucometer I've been offered. I have three waiting for me so I can test the accuracy of them, which I'm sure she'll love me for... I did tell the vet she hadn't been eating but am I right in thinking that if her blood sugar had gone back up overnight she must have had some food? I will try again with getting some water into her. She just let it dribble right out of her mouth last night, looking straight at me - normally she will take her medicine like a lamb. Her breathing looks much more normal than it was last night or this morning. If I can't get some more food into her with a baby syringe I'll try some honey.
 
She's up and eating! How do they go from one extreme to another like that? A good three teaspoons of Purina and now she's lying down looking more normal. OK I feel like I can go out for this glucometer now. I'll update as soon as I can.

Was Lily tested for DKA?
DKA = not enough food + not enough insulin + something else going on (possibly an infection or inflammation)

Thank you, I just got back and will read that when I've figured out this monitor. I didn't get enough blood out of her with the tiny lancet first go but it woke her up enough that she got up and had some lunch. Great strides for a little cat!
 
If you warm the ear first it should be easier to get blood. If you dont have a "rice sock" just get a washcloth and run it under hot water then place in a baggie use that to hold against her ear. Make sure its not too hot though

what is her favorite treat? We condition them with treats
take kitty to her testing spot give treat ...release repeat.
Once she stays in that spot fiddle with her ears...give treat release. Get where I'm going with this ? With every action she gets a treat . you want to have her associating testing with good yummies to follow.

It could be that the lancets are too thin. Using lancets that are 26-28 will give you a better chance at a large enough sample. 30+ gauge lancets are thinner. I'm glad she's eating more.

I get it! She's very good and tolerates pretty much anything but bribery will last longer than her patience!

33 is too thin. dang it I should have told you a lower number gauge.

33 are extremely thin. This may be your issue. Also rub or warm the ear first. I have a video in my signature showing how I test my cat CC at home
 
Great, thank you. And don't worry, I just took what the lady gave me. If I have to sterilise a needle this time to get a decent drop that's fine. Every time I let her go she goes back for a bit more food, I couldn't be more hopeful! Will watch that video now.

Thanks! She's my tiny little pal, and she even purred for me day before yesterday because I was crying, even though she felt so poorly. She always comes and sticks her face in mine if I'm crying and waits and tolerates substandard stroking (where usually she would give a warning snap) until I stop crying and then she goes off to do her own thing again. Not how I imagined a cat would be at all. So yeah, I owe her!

Sorry I seem to have missed this one! Thank you, I am in the UK. She was checked for ketones and was luckily OK. I have urinalysis sticks at home as well so I can keep an eye out for that. They were happy that she didn't have an infection, which was what I was worried about. I will measure her ketones as soon as I can get some blood out of her.

She is now sleeping in an actually normal position (she's been crouching a lot and looking generally like she's uncomfortable) and for the first time in a couple of days her eyes are actually closed. I've been treated to the sight of her third eyelids with her outer lids still kind of open so she just looked like a zombie. She seems to feel more like a cat. I'm going to go to the pharmacy now and ask for some bigger lancets.
 
its sounds like too much insulin for her. Caninsulin hits fast and hard you want to have food onboard at least 20 mins before injection.
Once you get those tests going you will know for sure. I think caninsulin last only 8 hours. I'm going to tag some caninsulin peeps to come help
 
Lord, ok. My main worry is hypo of course. I'm hoping once she gets some weight back on her it'll be less of an issue.
 
She normally had Harringtons dry chicken and rice, and that is down for her, but I've got some Animonda Carny and Bozita for her in the hope that she can be managed by diet at some point
