Gave 2tsp LC at +2 but went down at +3. Gave another 2 tsp LC so will test again in 30. Might need to break out the MC tonight.

11/10 History:
  • 9/11-9/13 vomiting, constipation, ketones small
  • 9/14 vet/hospital, started ondansetron + famotidine, "not DKA yet"
  • 9/15 low number event
  • 9/16 reduced from 1u to 0.5u for 2 cycles
  • 9/17 increased from 0.5u to 0.75u for 2 cycles, vomit
  • 9/18 reduced from 0.75u to 0.5u for 6 cycles, last day ondansetron
  • 9/21 increased by 0.25u to 0.75u for 8 cycles
  • 9/25 increased by 0.25u to 1u for 6 cycles
  • 9/28 increased by 0.25u to 1.25u for 6 cycles
  • 10/1 increased by 0.25u to 1.5u for 6 cycles
  • 10/4 increased by 0.25u to 1.75u for 6 cycles
  • 10/7 increased by 0.25u to 2u for 6 cycles
  • 10/10 increased by 0.25u to 2.25u for 8 cycles
  • 10/14 increased by 0.25u to 2.5u for 8 cycles
  • 10/18 increased by 0.25 to 2.75u for 10 cycles
  • 10/23 increased by 0.25 to 3u for 6 cycles
  • 10/26 increased by 0.25 to 3.25u for 6 cycles
  • 10/29 increased by 0.25 to 3.5u

Hi! Yes, I checked for ketones today and he was negative. Just tested again and he's at 46 so I gave 2 tsp of FF gravy lovers beef 20% carb. Would you call this MC or HC?
This is his first time below 50.

20% is high carb and the correct thing to do with BG 46.
I would test again in 20 minutes to see the numbers are heading back up.
Congratulations KC has earned a reduction in dose down to 3.25 units starting at the next dose, providing KC is high enough to give the dose.
 
+4 is 69! When should the next snack & test be?
Reading Tilly's Diabetes TR Protocol and confused about the reduction. It says:
"Alternatively, if the nadir glucose concentration is 40 - <50 mg/dl at least three times on separate days, try lowering the dose. If the cat drops below 40 mg/dl once, reduce the dose immediately!"
Since he was at 46, should I wait for him to go between 40-50 2 more times before giving a reduction?
 
You can switch to LC. Test in an hour.

For reductions, this is what we follow here for TR:

Newly diagnosed diabetics - less than one year since diagnosis
  • reduction *earned* after a single drop below 50 mg/dL and following TR, or
  • reduction *earned* after 3 drops between 40 - 50 mg/dL on three separate days for kitties who have shown they do not hold reductions well and following TR, or
  • reduction *earned* when the cat regularly has its lowest BGs in the normal range of a healthy cat (50 - 80 mg/dL) and stays under 100 mg/dl overall for at least one week and following TR.

If KC is a newly diagnosed cat (ie under 1 year since diagnosis) a reduction is earned after One drop below 50.
The three drops below 50 or one under 40 is for cats who have been diagnosed for more than 1 year.

I would give 1 teaspoon of low carb now and test again in 30 minutes to see if KC is managing to stay higher than 50. How does that sound?
 
Ok I gave 1tsp LC and will get a +7! I have an interview in 8 hours so I'll nap and set an alarm for 1 hour. I'm so happy with his greens! It's been almost 2.5 months since diagnosis and was beginning to feel a bit down but he's indeed so much healthier since then.

Don’t feel down!
If you think you won’t wake up with the alarm it would be better to leave food down for KC to snack on, although a +7 would be better;)
