INCREASE dose after 6 days on Lantus?

Any input appreciated. To me it looks as if the Lantus is building in Oliver and gradually PUSHING the high BG's further out in time (i.e., increasing duration). But the BG's still are too high and Oliver had ketones in his blood on Nov. 5 in the hospital. So, I'm thinking either increase 0.25 or 0.5 (per protocol bcs of ketones).

Oliver's vet just advised to increase to 1.5 units Lantus bid.

OK. Vets usually don't like to do 0.25U increments, so that's part of the reason for the larger increase. I think I'd still probably do 0.25U at a time with him still on a relatively low dose (0.5U from 1U is a pretty big jump), but also consider increasing after six cycles of 1.25U if necessary, doing the faster pace because of the ketones. That is the usual pace for the so-called "Tight Regulation" dosing method in the Lantus forum. The ketones and Oliver's recent hospital adventures make it hard to follow either dosing method exactly, but if you read through the info on TR it will give you an idea of what pace of change is possible and the testing requirements necessary to do a fast-paced dosing method safely.

Looks like he's on his fifth day of 1U? (we start the count over after a skip, so his time in hospital doesn't count).

I think the depot has had a chance to settle. With the recent ketones, if you don't see movement in the next cycle or two, then yes, I'd increase by 0.25U.
