? 11/10 Mouse AMPS 246: Bouncing? Need dosing help

Mouse has been on prednisole for about 2 weeks now, her numbers are all sorts of wonky. I could really use some help. I upped her to .5 to counter the initial levels because I wasn't getting any response at .25.

Maybe it's time to drop her back down? I'm resisting upping her dose because she's only super high first thing in the morning so I'm suspecting her dose is too high again.
 
How much pred is she getting, and what time of day? Usually pred means more insulin. Looks like Mouse could use an increase to 0.75 units.
 
I'm not sure of the pred dose, and I'm not home to check, but I give it to her twice a day, the same time as her shot, at 6am/pm, same as feeding. I won't be home until shot time.

See, I thought that too, until she dropped to the 91's and I was like "huh??" She'll have days where her numbers are great, and I think I don't have to make a dose change, and them BOOM! sky high again. And this coming off of her almost being in remission. It's so frustrating.
 
It's a bit ECID how fast cats react to pred. You don't have to split the pred dose to every 12 hours, you can do just once a day. I remember one member whose kitties numbers started rising a couple hours after the pred dose, so she gave the pred in the morning, so kitty would be safe while she was away at work. Some experimentation may help you fine Mouse's pattern on pred.

Since you are following TR, nadirs in the 90's mean you have room to increase. Often when kitties come out of remission, they can be a little bouncier that the first time on insulin. It's also important that you not hold doses too long, as you have been. The faster you can get her regulated again, the greater the likelihood of remission again.

I haven't been able to get her regulated on the pred which is why I've stayed at the .5 so long. Today was my first chance to reach out for help too.

Mouse tends to only spike bad over night, so I'm not sure when I would give her the dose, if I were to combine it. I'm only "working" outside the house 2 days a week right now, during the day.

I'm not sure how much good changing her dose right now is going to be either, since she'll be having surgery soon for her biopsy, and they're just going to muck everything up again on me.

I'm sorry. I'm just feeling very defeatist right now.

I'll raise her insulin tomorrow, and try giving her both pills in the AM? I'll try to be more active in the forums so I can get advice on her dosing, because I think I'm out of my league with the addition of the pred, and I'm frankly overwhelmed with this new health issue
 
If she is having surgery for the biopsy, then you'll have to have her off of pred for a few weeks before that. Otherwise the pred can make it hard to diagnose what you are dealing with. I think it's worth an experiment doing her full dose in the AM and see how she does.

As for the IBD or cancer question, once you have an answer, the path forward will be much clearer. :bighug: Been there, done that, three cats now.
 
So this is what I just got from the vet about the pred: "We are going to start weaning Mouse of her prednisolone in preparation to try and get intestinal biopsies. Decrease her dose to 1/2 tab twice daily for two weeks, then 1/2 tab once daily for two weeks, then 1/2 tab every other day for two weeks."

I think given this, I'm going to hold off on raising her insulin for now. The goal is to have her weaned off it by January so she can have the biopsy done. I will try giving her the doses as a single dose instead of split into 2 though. Those spikes are scaring me.
