Sept 30 - Charlie - dosing question

So a little bit of history. Charlie was diagnosed on September 1 and started insulin shortly after. We started at two units and increased to 2.5 but his numbers seem to be increasing as well.

somebody in one of the Facebook groups messaged and said that this could be an indication that his dose was too high. So I dropped him to 1 unit and he seem to stabilize around 350-425. High, but no big bounces. So I increased him to 1.5 units after a week. And now it seems that his numbers are all over the place.

I don’t know which Facebook group you are using for advice but I think it was not at all correct. There’s no indication that he received too much insulin. He never dropped under 90. Please read the SLGS method again. Hold the 1.5 dose for a week and then do a curve unless a reduction is earned by dropping under 90. Lantus dosing is based upon how low a dose takes your cat. If you can get before bed tests at night that’s great. A +2 often used an indicator of whether it will be an active cycle. If the bg is lower than the preshot, you need to test more. As of now your ss shows it was not needed.

I had read this but their but I get conflicting advice and it’s so confusing :(:( she basically said that because his +6 was higher than his AMPS, he’s getting too much and than sometimes increasing too quickly (or starting too high) can cause their BG to increase as well.

I plan on increasing him to 1.75 (or maybe 2.0) on Friday

I have no issue setting alarms to do testing whenever it’s needed. What do you mean by “active cycle”.

And you mean if his BG at +2 is higher than at PMPS I need to test more frequently throughout the night?

No. I meant lower. Lantus usually kicks in around +2 for most cats. There is more overlap between cycles sometimes. So if the +2 is LOWER than the pretest, there’s a greater chance that your cat’s bg might decrease a lot so you need to test more to make sure the bg doesn’t drop into hypo numbers. Are you using a human or pet meter? Please add that to your signature. It looks from your spreadsheet like a human one. So the take charge time is if he gets close to 50.

Sorry, I meant to write lower and wrote “higher”. Long day lol.

I will keep that in mind. Tonight pmps was 21.7 and +2.5 was 18.4 so I will test him again in a couple hours
