I was asked to take in a diabetic foster cat back in April 2020. I was told he was on a raw food diet & was on supplements and I would be doing home testing: all that would be provided by the rescue. His foster home then wasn't viable, he was stressed out by one of the other fosters. I decided to give it a try.

When I went to pick him up a few days later (4/11/20), I wasn't given a meter to be able to home test. I couldn't start testing until 4/23/20 when the meter arrived (I know now I could have purchased Relion). Another week went by until I spoke with a Vet that helps the rescue. She made sure I knew how to home test correctly and gave more supplements. Then she brought up insulin, I was never told he needed it and his online profile said he was off of insulin (that was before the stress at his previous foster home).

A few more weeks went by until I received insulin & syringes and he was back on insulin starting 5/15/20. I know Vetsulin is not the best insulin for cats but it is what the rescue can afford to provide currently. His numbers for a while just seemed random, I would do a control check to be sure things were working. Recently, his numbers have been more consistent and improving. It was suggested I do a curve but have yet to to do so.

I was originally feeding 2.50z of Darwin's each meal, but was advised by the Vet to cut down to 4.3oz a day split into 2 meals. But he always seems hungry and searches for food and crumbs everywhere, I had to lock down the kitchen. Another diabetic cat foster within the rescue mentioned she was told not to reduce until after his numbers were under control. Should I also wait or should I keep on the lowered amount of oz each meal?

I was told to join the forum to get support and information needed to succeed. This thread is to give some background information, of what I know. I can check to see if there is anything else in his history to provide that info. Any advice would be appreciated or corrections to the current path we are on.
 
An uncontrolled diabetic is ravenous. PLEASE feed him as much as he wants. Once the insulin starts working you can cut the food back to 2 meals and a few snacks in between.

We are GLAD you found us and it sounds like you are getting good support from the rescue. THAT is AWESOME!

I noticed there are no tests in between shots. These tests are just as important as the "before shot" But I will let the experts guide you on that.

Yes agreed! Feed feed feed. Diabetic cats cannot process the nutrients in the food properly so they eat and still lose weight and that’s why they’re always hungry. They also lose calories just by peeing since there’s sugar in the urine. For now just feed him as much as he wants. Even after diabetes is regulated, I don’t think 2 meals a day only work best for most cats who are grazers. Most of us here feed the bulk of the meals around shot time and then a few smaller meals in between.

Btw, did the vet happen to mention if he’s under or over weight and what’s his ideal weight?

I’m not familiar with the meter you’re using but it’s great that you’re testing the preshots. Are the test strips expensive and who is paying for it you or the rescue? Just curious because it may be worth switching to Walmart’s ReliOn since the strips are so affordable. It would be great to have some day tests sprinkled in there too especially at +2 and +4 since Vetsulin hits fast. I can’t advice on dosage because I’m not that familiar with the insulin but I do think you may be able to give a token dose instead of skipping so much. Tagging some more folks here because I’m not sure who is the best at Vetsulin @Nan & Amber (GA) @Deb & Wink @Bron and Sheba (GA) @JanetNJ
 
OH, the preshots are looking MUCH better! Please do get some mid cycle tests to see how low he's going. Vetsulin tends to be at it's lowest at 4-6 hours after the shot... so getting some test in during that time is good.

Feeding... the rule of thumb is about 20 calories per IDEAL weight. so I don't know what the calorie count is on that food, you will have to read the label. So say he's 10 lbs and he's not over or underweight, he would need ABOUT 200 calories a day. NOW, that being said, diabetic cats often need a little more then that because they don't process nutrients as effectively. I see that he's also got CKD because you are giving phos binder. If you have a copy of his latest labs, there's a labs tab in the spreadsheet where you can enter that info.

We were wondering about feeding. I feel that it's best to go that route. Because he is always searching for food. I started feeding more these last few days. Thanks for your advice.

Thanks for the advice. I've started feeding a bit more each meal and will continue adding smaller meals as well. I will update the signature.

Not sure on weight. I believe she said 8 or 9 pounds at his last visit. The rescue pays for all the supplies. They chose the meter. I did buy the Relion meter last week when I ran out of strips. (They order online) The other diabetic cat foster and I suggested we switch to using that instead, we shall see if they want to. I will start testing a few hours after the shot to get those numbers logged.

I will see if I can get a copy of lab work to add. I will start getting mid cycle numbers this weekend. Thanks for the advice.
 
With Vetsulin, it's extremely important that you get more tests. Vetsulin can drop blood glucose (BG) numbers quickly and quite low so the risk for hypoglycemia is greater than with some other insulins. (Vetsulin is not recommended for cats. It was developed for dogs but some vets use it with cats.) Also, are you feeding Professor about 20 - 30 min before his shot? You need to have food on board before giving a Vetsulin shot.
