? 5/30 Brody AMPS: Could use some input Uing Levemir

Hello,

So due to some failures on my part I ran out of insulin and missed 3 injections. I was able to get some insulin and gave Brody his first injection last night at 7:30. At the time of his injection his bg was 495, that morning he was 414 so I assume he was 400+ all day. This morning at 7:30 when I was going to give him his shot he was at 90 so I decided to wait and see his numbers, 30 minutes later he was at 77, 30 more minutes 78, 20 more minutes 78, 40 more minutes 72. I would not have predicted that the insulin would have acted so quickly and now I don't really know when I should give him his second dose or why the reaction was so quick. Any input would be appreciated.

Sorry that no one has responded to your post until now. When asking for input, it's very important for people to be able to see your spreadsheet as the information that will be given is based on data.

If you can get your SS up and running that would be very helpful.

Just as an aside: Levemir has a later onset nadir that Lantus. Freckles gets Levemir and her nadir is anywhere from +8 to +12 so I had to get used to shooting low numbers. Secondly, sometimes cats numbers continue dropping when food is withheld. Third, insulin is not as interchangeable as one might think. When a caregiver wants to switch insulin, it's best to post and ask for help because often it's best to decrease the dose of the new insulin when first switching over. You never know how a cat will react to a new insulin. Fourth, with Brody having a history of DKA, it's EXTREMELY important to continue monitoring for ketones and to make sure Brody gets insulin and food. No insulin and you are at risk for ketones again.

No worries. I had stopped using the spread sheet a long time ago but it might be best if I did get back in the habit. I have been using Levemir since last December I think. The only other variables that have changed is I had to get insulin from a vet and I used difference syringes, the needle being much longer that the ones I typically use, not sure if that matters. His numbers are still really low but do appear to be rising now.
 
So I am also a bit confused. The vet prescribed Levemir U-100 but gave me syringes to be used with U-40 insulin. Does this really matter?
 
Yikes! Yes that really matters. It has to do with the amount of insulin that is in ml.

I'll see if I can find more info on that. Don't shoot any insulin until this gets straightened out.
 
If you are using the same markings on those syringes, you are giving two and a half times the dose!:eek::eek::eek:

Michael: I use Levemir and these are the type of syringes I use. They are u-100 and have 1/2 unit markings. You can get them at any pharmacy and you should not need a prescription. They have a nice short needle.

Okay, I updated the permissions for the spread sheet.

I was trying to understand the difference between the syringes. I get that the U-XYZ rating is how many units of insulin is in 1ml but I don't understand what is different between the syringes. I have an old box of U-100 ReliOn syringes I can use but they both say 3/10ml but I don't know what that is in reference to. I feel like a dummy right now and I am almost certain that either the vet or the staff at this place is incompetent based off my limited interactions with them. So if I understand I actually gave Brody 3 units of insulin last night...

I have had a similar interaction. Some vets have more training on diabetes than others. It's really the consumer who needs to get educated because at the end of the day, its the caregiver who has the most to lose.

Glad he came up and is OK. Stick with this board. The experienced members on here have lived and breathed FD 24-7. Ask as many questions as needed so you have a clear idea what you need to do.
 
When you have a chance please update your title to include the date and kitty name; AMPS. I switched to Lev last July - much easier on those bounces that Luci was so fond of.

Hope you can get your SS up and running and fill in as many blanks at you can. It really helps to keep it up...I have to write Luci's numbers in a book and then transfer to the SS when I sit down at my computer - like now...not well today and didn't even post yet...bad me.
 
Both Lantus and Levemir are U-100 insulins. It's a matter of how concentrated they are. The syringes are 3/10 CC. See how they go up to 30 units? Each of the 10 units is 1/10 of a CC.
 
I can see your spreadsheet, thank you. :) Can you change that 1.5 units last night to 3.75 units (times 2.5), cause that's how much you really shot. If last night was your first dose of Levemir, could you also put a blank line after the day before, and put something like "start Levemir" on that line, so we know you switched. We also typically switch at 70% of the Lantus dose. So your Levemir dose should have been 1.0 unit. You may need to increase closer to the Lantus dose, but some cats react strongly to the change, so we do the bit of a reduction for safety.
 
Last night wasn't the first of Levemir, I just missed the previous 3 doses from not scheduling my insulin restock appropriately. I have updated the sheet htough.

I am confused on the syringes because both packages say 3/10mm so I can't tell a difference based off any metric on the package but the markings on he U-40 syringes are definitely larger and more spaces out. They both have half unit markings too. I should be able to get a lot more data tomorrow so we will see how things go.
