SLGS Method - Consistently <150bg preshot -Not keeping weight on -

Hello!

My kitty Cosmos has had diabetes for a few years now. He is on Lantus currently and has been under 150 consistently for 10 days (mostly lower 100s or higher 90s). He had one morning that spiked and he did get a shot. He gets fed 2.5 to 3 cans of Dave’s Pork Restricted Recipe... chicken and beef make him sick. I have noticed that while his numbers have been pretty great and we haven’t needed to give him Insulin based on the BLGS method, that he hasn’t been really keeping the weight on and his appetite is insane even if he gets 3x what he should eat at his weight. He has had testing for hyperthyroid a few times in his life and it has always come back negative, he does have an enlarged heart, and has had mild kidney issues before. Currently, he is on antibiotics for one more day for a possible UTI as it isn’t fun for him to clean after he poops (sometimes it cause one of us is near him or the dog is while he cleans each time after he goes... which is what made us take him to get the antibiotics in the first place. But, yeah, super hungry all the time... I could try feeding more food... like.... 4 to 5 cans a day but I just feel like... is there something else I can do? Or maybe something I haven’t tried that has worked for others? Any words would greatly be appreciated! It does feel good to say his numbers have been consistently manageable on their own with no shots with that below 150bg preshot!

When looking for remission it is best to get a strong remission because Once a cat falls out of remission it is much harder to get them back into remission the second time around.
Therefore it is best to go down from 1 unit to 0.75, then 0.5 then 0.25, then 0.1 then a drop.
That way you are giving the pancreas time to heal completely and hopefully have your cat stay in remission for the rest of its life.
If I were you I would start back on say 0.25 units twice a day and see what happens. I would also test during the cycles because Lantus dosing is based on the lower point in the cycle (nadir).
At the moment you have no idea what is happening during any of the cycles at all. I would get one test in during the am and the pm cycles as well as the preshot number. And remember cats often drop lower at night.

Have you tried adding water to his food so it fills him up a bit?
How many calories does the food you are feeding him have? Maybe he needs a few more calories in it as opposed to amount of food.

I will try the water thing. His food is 184 calories per can and it per Dave’s feeding recommendations, it says he should have about 1 can a day or so for his weight. Ideally a good weight would be 11 lbs for him I feel like.. not the 9.3 or so lbs he was at the vet last week. He ends up getting 542 calories a day for 3 cans... of it is more of a 2 can day... 468.

I managed to get in one curve so far and a couple spot checks (that can be seen on “Cosmos’ Spreadsheet in my signature section. I also checked his ketones this morning (those are all good). I’ve been trying the .25 but he had a higher evening one night so I gave .5 that particular night. But we are working on seeing how that goes. Plan is to do 1 curve a week and go from there based on the data. Thank you for the info thus far!
 
Hello! I have tried giving .25 regularly so that we aren’t just dropping off with the insulin. He has been pretty stable which is awesome. If the number is below 80 (or 81 really.. it’s so close to 80), we have beee seeing he dips down a good amount on his own. Please check out his spread sheet! It’s updated to the date of this post! I have also been doing a curve once a week, minimum and occasionally checking for ketones.. which he’s had negative for ketones. :)

He is still ravenously hungry most of the time. He’s been tested throughout his life for pancreatitis, thyroid issues etc.. and nothing ever comes up in the panels. (Dude likes to pee/mark stuff too... 3 areas basically... and the box.... just trying to do what I can with just cleaning it up the best I can. It doesn’t happen daily - but he will have bouts where it happens a whole lot - than off and on. He has one spot outside the toilet he pees in regularly - that I have just put a little plastic trough so I can just dump the pee.

Hope everyone is doing alright and thank you for checking out the chart! :). I guess I just wanted to check in since I posted above a while ago and kind of get an idea of what direction I need to go in - or just maintaining where we are at until we MAY not have to give him insulin (I think we always will - but I can hope for remission, right?)
