Incredibly frustrated

My cat is a year and a half old and was diagnosed with diabetes the first week of January. He has been on Lantus 3 units BID for the past month and his fructosamine bloodwork came in today at 590. I'm absolutely frustrated because last month when he was on 2 units his fructosamine came back at 500, so I was sure we were getting close to his "golden" dose. When he was on 1 unit, his fructosamine was around 520.

He has been seeing an internal medicine specialist at UC Davis and we were just recently told that it may be impossible to get him into remission because of how insulin resistant his body acts. We have tried so many things... Freestyle libre sensor would only last about 6 days (we had the sensor reapplied many times) and his glucose levels never dipped below 430, he would still urinate large amounts on Vetsulin (my vet informed me that this is because it is a short-lasting insulin), and his body would not respond at all to the Toujeo insulin pen (so we switched to Lantus with U-100 syringes).

Doing glucose curves on him at home or at the vet is out of the question because of how aggressive he is. I used to work as a veterinary technician and he is, honestly, the worst patient I have ever had. I have to give him gabapentin in order to even take him to get his blood drawn for the fructosamine.

Honestly, it seems like he has been doing great on the Lantus 3 units BID. He was always stuck around 9-10 lbs and yesterday he was weighed at close to 12 lbs.

Has anyone had to deal with such a frustrating situation? I'm wondering if anyone here has ever just ignored bloodwork results completely and to just focus on their clinical symptoms/behavior at home for finding their correct dosage.

WELCOME TO FDMB! The absolutely the best site on this planet to help you help your Schmeow? is it? How did the vet increase his units to 3? Was it by increments? We get many new member that are giving high doses because their vet increased by full units, many times missing that "golden" dose. I am no expert but am a veteran of this site. So I wont advise on dosages Lets see what others say ok?

I'm guessing you wont be home testing. I remember when I couldn't test. It took two of us but we finally got Trouble more use to the "program" and while he fought us... He tolerated us.
A lot of it had to do with conditioning. During a down time rubbing his ears. So many little tricks that involve treats. He got a treat when we failed he got 2 when we were successful.
I think maybe you will find others here with the same story. We are all here paying it forward. I hope we can help you.;)

I can relate because Minnie’s diabetes was unregulated for over 6 months. The vet kept increasing the dose and at some point we thought she was insulin resistance too but it’s exactly what JT said, increasing by units may completely skip the right dose for your cat.

Another thing to consider is trying a different insulin that he may respond to better. I know a lot of folks here have done that. I got lucky with Lantus but was considering switching when Minnie finally got regulated. And honestly, I think is was blind luck because I’ve been able to bring her down from 9units a day to under 6 by home testing and decreasing in .25 increments, which means she was being overdosed before.

I don’t know much about the fructosamine other than it’s a 3 week average so I’d think it’s not the best way to determine the BG levels. They need to be monitored daily and a few times a day actually. I only had it done once and all the other times, her dose was based on behavior and BG levels. Let’s see what the more experienced folks will say so hang on! You’ve come to the right place and these wonderful people will help you with guidance and options :cat:

What dose was he on? Toujeo is just Lantus (Glargine) in a more concentrated solution Lantus is U100 (100 units per ml) while Toujeo is U300 (300 units per ml).

I think that would be dangerous since if BG drops too low it may be too late to prevent damage to the cat.. One could use a urine dipstick to measure glucose and just make sure that some glucose is in the urine. Otherwise if no glucose one does not know how low the bg is. Glucose spills into the urine with BG is >180-240 or so.
Haws complete blood work been done? I ask since normally a cat 1 1/2 years old does not become diabetic. Maybe there are other problems too.
