I was in another work meeting and failed to give Maxi his +6 R. His number at +5 was 368. Should I follow the scale at PM shot time as below?

6 units of Lantus + R like so:
PS 350 or higher, 1.5u R

PS 300 - 349, 1.25u R

PS 275 - 299, 0.5u R
