We did our first curve today and his numbers were all over the place! They usually stay in the 200-300 ranges. Can anyone take a look at his spreadsheet and offer dosing advice to start with tomorrow?

Looks like Oswald started off by having a very nice curve down to 123 then freaked out at the 'low' number and decided to bounce.
Bounces can happen when a cat drops too fast, too low or lower than he is used to being. He then dumps regulatory hormones and stored glucose into his system to save himself and that sends his BG shooting up high. Bounces can last from 1 to 6 cycles (3days) and there is nothing you can do about them but wait them out. Bounces are normal especially in newly diagnosed cats.

SLGS says that " if the nadirs are between 90 and 149 maintain the same dose"
Oswald has only had one nadir under 149 and that was today, but I would stick to the same dose for a few more days and see what he does when he comes off the bounce. If his nadirs remain up over 150 after the bounce clears I would increase the dose.
Post and ask for advice any time.

Ok so tomorrow I’m going to have to be gone for several hours, do I keep testing him every couple hours until then? And then when I get home try to test him often? Basically try to keep the curve tests going as much as I can for the next couple cycles...

No, with SLGS you only need to do the one curve, but you can test more if you want.
Getting tests in the middle of the cycles to see if you can catch the nadir to see how low the insulin is taking Oswald is very helpful.
Dosing is based on how low the insulin goes, not the preshot, so getting nadirs is very helpful.
You are testing enough to do TR if you wanted to.

Oh ok. I don’t want to test him more than I need to. I feel bad for his lil ears. I just didn’t know what’s best. I’ll keep going the way I have been in previous days. Thank you very much!

Hey Kortne!! I agree with Bron…..I think since Oswald bounced so hard today, better to hold the dose for a few more days and see what happens when that bounce has had time to clear (usually within 6 cycles)

It may also help for you to start posting in the Lantus Forum...everybody there uses the depot insulin's so you'll get the most experienced help there.

Here's some information on Making the most of your Lantus forum experience that talks about how we post there.

Hey Bron, I noticed that Oswald still has some kibble everyday, wouldn't she have to take the kibble away completely to do TR

Am I not doing SLGS? I read about both strategies and figures SLGS was geared toward what we were doin. Once I get my job back there needs to be some dry food or no food during the day for about 10hrs. That’s what I’m concerned about. I’m going to have to do amps and leave about an hour later, which is when I would leave him some kibble. I’m going to go buy dr elseys chicken kibble today. He only gets about 1/4 cup between amps and pmps and usually doesn’t eat all the kibble.

Yes you are doing SLGS. I just commented you were testing enough to do TR but hadn’t realised you fed dry food (which Diane pointed out) which means you can’t do TR. If you need to feed dry because of your working circumstances, that is fine. No problem.

Oh ok good. I really just wanted to see what’s been going on with him which is why I tested so much. I think I’ll give him a break though until next curve and only test at amps and pmps. Thank you for clarifying, I was afraid I was doing something wrong!

You are doing a good job with the testing. Don’t be discouraged because he is bouncing. A lot of cats bounce. I would continue to get at least one test in during the cycles if you can so you can see 1. How low he is going with that dose and 2. So you can see when he comes off the bounce.

You might want to move to the Lantus ISG forum where there are lots of experienced lantus folks to help you out.
