Hi, my sugar kitty, Bandit, is on lantus and I usually post on the L&L forum but I needed help interpreting the renal lab work for her civvie sister, Budge.

Budge is 12 years old and healthy (or so I would like to think). A tad overweight at 12 lbs. Has been on kibble all her life and is not much of a water drinker. 3 months back she showed signs of what I thought was UTI (straining to pee, running to the litter box several times to pee but either not peeing or peeing very small amounts etc). I took her to the vet and after physical examination he said that her bladder was not distended. He gave her shot of something (would not tell me what it was) and asked me to add a pinch of soda bicarb to her food twice a day for 3 days. She was fine after that.

But I took that as a wake up call and began to give her wet food in addition to her kibble. She seemed fine till last night when she didn't pee. This morning she went to the litter box to pee but nothing would come out. I took her to the vet right away and it turned out to be a urethral obstruction. The vet managed to clear the obstruction and took blood for lab work. He has again advised 200mg of sodium bicarbonate once a day for 3 days.

Budge's renal values have come back significantly higher than normal. I am told that an obstruction can elevate values. Also read that higher values could also be on account of insufficient water intake.

Would be grateful if someone could look at her attached lab work and tell me if there is anything I need to worry about? I have already started transitioning her to a completely wet diet to ensure she gets enough water.

The 2 people that I know are the best at interpreting labwork results are these 2 people I have tagged below.
@Marje and Gracie
@Larry and Kitties

It's absolute madness here in the US, with the covid19 outbreak. So it may take some time for a response from people. Everyone is upending their life and trying to juggle 10 times as many things as before.
Please be extra patient. Thank you.

p.s. Stay safe there in India.

THANK YOU. No rush. It looks like the lab have put in canine reference ranges in the report! :banghead:
Not the first time they've goofed up the lab work of my kitties.

Will upload the corrected report as soon as I get it.

Hi sorry to hear about Budge diagnosis, you may want to ask them if the values are the correct ones for her, hopefully it was all a mistake .

But if not her values even though they are a bit off pointing to her having some kidney issues they are not that bad and there are some extra questions you may want to ask

  • Did they they tested for phosphorus levels ? ( it is not on the test results and is very important to know how it is is one key issue you want to monitor in a cat with renal issues)
  • Is her blood pressure ok ? ( some cats with renal issues tend to have high blood pressure and you need to address that if that is the case)
  • Did they tested for potassium levels?

I am back but still fighting jet lag and a cold (only a cold; no other symptoms so I’m good). If you can just tag me when you get the new labs in, I’ll take a look.

Please stay safe there.

Hi @Marje and Gracie, attached the revised reports for Budge. The idiots in the lab had put the reference ranges for canines. (This is not the first time that I have had a lab screw up a test report!)

Budge's renal values are elevated - I am hoping that was on account of her urethral blockage - the blood sample was drawn soon after the vet cleared the block and drained the urine. She hadn't peed in 24 hours.
She also doesn't drink enough water.

There is no rush on this. This can wait. Please take care. Whenever you are well enough is fine.

Sorry for the delay.

There’s really not enough info to go on. Yes, her creatinine and BUN are elevated but whether it’s early CKD or not is not possible without more info, especially because she’s had a blockage. When we look at kidneys, we also look at the urine specific gravity, an SDMA Test if available, calcium, phosphorus, potassium, PCV (hers is normal).

I’d suggest having a full superchem, CBC, urine done not more than three months down the road.

thanks @Marje and Gracie for taking the time - especially when it's so crazy all around! :)

will need to check which of these tests are available here in mumbai. a serum electrolytes test is available which tests sodium, potassium and chlorides in the blood and of course, a routine urine exam. will check on SDMA, calcium and phosphorous - i presume these are blood tests and not urine?

Yes they are blood tests but it’s very unlikely they can do an SDMA. That is usually only run by IDEXX labs. I’m sorry I forgot you were in Mumbai..

Moey went into acute renal failure due to a urinary obstruction. He was hospitalized for 4 days with a urinary catheter(he removed himself after day 2) and IV fluids/Meds. It's been a while so I can't remember specifics(over 10 years ago). Moey miraculously recovered with no permanent damage. He was on a special diet for a few weeks(Science diet K/D) for the kidneys then transitioned to a urinary diet(Science diet C/D multi). His kidney values returned to normal.... I can't exactly remember the time frame but, it seems like it was within a week or 2.
Hang in there Bhooma. Hopefully Budge's elevated values were due to the obstruction. Paws crossed after a few weeks, she'll get back into the normal range. Sounds like you're on the right track by switching her to canned food. If you're not already, consider using bottled water. I use distilled because it has nothing in it. I'm leery of any water besides distilled. Heck, it seems like most bottled water is tap water in fancy packaging.

Prayers for y'all :bighug:
