2/12/20 Dx 11/19 when to decrease or resume regular dose

My boy has been getting 3 units Lantus twice daily and finally seeing better numbers. I held dose last night and resumed normal 3 unit dose this morning and checked numbers every 3hrs. Should I have given him smaller dose last night and attempted reduced doses today? Obviously he would be higher this morning after no shot and safely get his 3 units but was that the right decision? He eats mostly wet pate with very small amounts of dry due to milk cat household. Especially since I’ve been feeding him 4 times a day to gain back some of the 4 pounds he lost since December! He looks better and it keeps him out of the dry foods.

Lantus is a depo drug so giving a smaller dose would not have helped prevent him from going too low. Which method are you following? I’m assuming SLGS. What you should have done last night was to stall, not feed, and test again in 20-30 minutes to see if he was going up. Then you likely would have been able to shoot the 3 units although I would not have increased by a half unit like you did as we usually increase by .25 increments. Had you stalled and shot you would then need to shoot 15-30 minutes later in the morning.

I think you have been at this long enough to maybe adjust your "no shot" number? You are raising the dose to get some nicer numbers- so what numbers are you comfortable shooting? I'd think anything over 150 is game, that's where we started but why even 175 would be better than skipping- especially if you are home to feed and monitor for awhile.

On 1/31 when you skipped that dose, the previous BG check @ +11 showed that your kitty was on the way up. Cats BGs can continue to rise even after the shot is given. Grabbing a test +1 or +2 can often show you this- so the next time you could I might grab one. This way you have information to help you see where your cat is heading.

This post explains some of these testing tips when it comes to filling in some of the other spots on the SS. In testing, a +2 and +11 can often be really helpful in making future decisions on when to shoot and where the cat will be heading afterwards.

With dry food in the picture, that means they earn a dose reduction if you see a number under 90. For now, stick with 3.0 units. Take a look at the Dosing Methods Sticky Note, under the Start Low Go Slow Method. There is a section on options if you are confronted with a lower than normal preshot number. Skipping is one option, so is a reduced dose if you can monitor. Gradually,when you have the data, you will be able to lower the number you feel comfortable shooting.

Since you cannot follow Tight Regulation, due to dry food, the Sticky Note on handling lower numbers is not applicable.

I have attempted to draw up .25 increments and it seems impossible and especially to do it the same every time. My daughter helps me with his feeding and shot 3 evenings a week and I don’t know how consistent she could be either. Do you think maybe I should try the decrease to 2.75 like I should have in the first place like you mentioned. I’m so used to the high numbers the good numbers scare me!

I tend to follow the start slow method but I increase in a little higher increments. I have a hard time being consistent with the .25 increments and have my daughter help me at times as well and I’m afraid we wouldn’t be consistent. I know my no shot number probably needs to be lower. I’m so used to high numbers the normal ones scare me!

Thank you for your reply. I will try to grab some extra checks at those times. May help me feel better about a lower no shot number.
