Simon was started at 2 units after his weekend in the ER, and has remained high ever since so it doesn't seem appropriate to start over at .5 units.
We wouldn't suggest dropping back to .5 anyway....when you switch from Vetsulin to Lantus, we take into consideration the dose you were on before so you started right where you should have.
I'm not sure if I should be adjusting Simon's dose. He's been at 2.5 for well over a week and his numbers remain pretty high
Yes, you can increase.....Don't hold a dose that's not getting the results you want for
longer than 7 days. If (for example) he was dropping into high greens, we might suggest holding it a little longer, but he's staying too high for too long. If you stay with a dose that's not working it just allows glucose toxicity to set in. (where the body gets used to those high numbers even with insulin).
I'd go up to 2.75 (do your best to keep it consistent...in between the 2.5 and 3 unit marks)
Do be sure to always get at least a "before bed" test on the PM cycle. If it's possible, you might want to get a +2 and a +4 (if that's how late you're usually up). Most cats go lower at night so it's important data to get.
