01/02/19 AMPS 210 -Inconsistent numbers

Caren & Apple

Member Since 2019
Hello- The question I'm posing to the group today is regarding Apple's continuing inconsistency in numbers on his spreadsheet. We've been getting highs and lows with no change in his diet, routine or anything remarkable. Is this to be expected? He's not lethargic, still eats well and otherwise normal, except those numbers.
Apologies if I'm posting this incorrectly...

Could you put Apple’s name in the subject line? Also, did you give insulin last night? Dose is empty in the spreadsheet. I am guessing you did, because you saw some nice numbers last night. However, Apple’s body isn’t used to those numbers and is now bouncing. Totally normal for a relatively recently diagnosed cat.

Thanks for responding. So I guess other than the numbers yesterday, total overall week after week, the inconsistency is to be expected?

A couple of housekeeping items. Like Wendy noted, please make sure you include your kitty's name in the subject line on your thread. Also, please drag the 2020 tab on your spreadsheet to the far left. That way your spreadsheet opens to the correct tab.

With respect to the inconsistency, some of what you're seeing are bounces. It will help if you follow TR since at this point, I'm hard pressed to know which dosing method you're using. With TR, you do not reduce the dose unless numbers drop below 50. You reduced on 12/25 and numbers did not fall into dose reduction range. Also, you evaluate the dose every 3 days/6 cycles. If numbers are not in the range you want (i.e., greens), you increase the dose. You've been holding doses too long. This allows glucose toxicity to develop which basically means that Apple is treating higher numbers as the new "normal." The aggressive nature of TR prevents this from occurring. You held the 1.25u dose for 17 cycles. I'd also encourage you to get a before bed test every night. With TR, the protocol encourages at least one spot check per cycle in addition to your pre-shot test. You don't know if Apple dropped into lower numbers during some of the PM cycles when you didn't test given what you saw with her cycle last night.

I think this may be your last post: http://www.felinediabetes.com/FDMB/...associated-with-diabetes.222911/#post-2492812

Caren, would it be possible for you to post daily as to how Apple's numbers are running? It's very helpful if you post daily so you can get advice, suggestions and support along the way. It's so hard when you're always trying to play catch up - such as an increase would have been due long ago - as per comments above.

Hope to see your next post tomorrow :) Have a good evening :)

We increased his dose under advice, then reduced it back to 1.25 after being admonished for raising it with that one green on l2/23. I'm really screwing this up and totally rely on people with experience whether here or an associated fb page. So with that said, everything you point out is true, so I'll get more pm tests and see where the numbers go. I won't raise until advised to do so and hold it for 3 days. So you suggest changing to TR and 3 day cycles, when does that mean we should increase? Immediately?

Absolutely will post numbers everyday and get pms. I thought I was posting too much now...as always thank you both for all you do.

Caren --

Have a bias so I want to be up front about it. There are only a handful of people who have experience and who post on the FB page. Fundamentally, you have inexperienced people offering other inexperienced people advice. There are no spreadsheets to look at the there is a good chance that most people who are looking at a spreadsheet don't know know to interpret the data. And just to further ice the cake, you have people using an entirely different type of insulin telling you how to dose your cat. And, as I'm sure you appreciate, Lantus is very different than any of the non-depot insulins.

I think you will do far better if you get your dosing recommendations here.
