I read your comments in yesterday's post about Chanel living in the closet before she came to you ... poor thing.

It's too bad that you have to work and cannot get a test mid-cycle...as much as no one wants to do this, I'd suggest you set an alarm (yes, I know...ugh)...to get up in the middle of the night and check her - at least once...and always get a test before going to bed...at least that way you'll have two mid-cycle tests in a 24 hour period...you may be missing out on the fact that she's getting lower a few hours after getting her insulin.

Sending vetty vines for Chanel today. It'll be interesting to hear what your vet has to say about her diabetes - I've learned a long time ago they know very little and I do not take their advice or discuss at length Luci's diabetes - this knowledge and experience of this collective group far outweighs 99.9% of vets as far as FD is concerned.

Thank you Sue for the advice. I think it is a great idea to do that and this way I can test in various days at different intervals in the night. I am still looking for a remote job because I feel the schedule I have now does not favor Chanel's health.
As for the vet, I am going because this is one of the few endocrinology specialists in diabetes and thyroid in Massachusetts and waited like 6 weeks to get this appointment. However, I will be very protective of Chanel, not letting them test her further than a T4 or fructosamine that were refused by previous vets. As for the vet experience with Lantus and diabetes, I will listen to what she has to say in the most polite way. I have no intentions in following any other advice than FDMB because hey, the proof is in the numbers and also the FDMB members got me to a place where I feel more confident than ever in handling Chanel's diabetes. The things I was reading for the past days on the forum connected all the dots for me. However, my husband and I promised ourselves that this is the last specialist we will see and if she starts with the acromegaly hypothesis, we are out of there.
I think Lantus would have worked from the beginning if I had the right instructions. Oh well, even now it is not too late and although Chanel's first BG curve was more of a straight line :), we can think of that as a result of not enough insulin to give a proper nadir (although she did have a lemon around +8).
Meanwhile, I ordered the supplement for raw meat and we will try see if Chanel might be interested in eating raw food and maybe, just maybe less dry food.
Since today I will work from home, I will get more BG values.

Now that's the kind of vet I would think has greater knowledge than the regular vets that most folks (like me) have seen. Excellent choice!

Although I'm not sure of the value of a fructosamine test at this point - your BG's that you've tracked are already a testament to what a fructosamine test will tell you - historically her numbers have been high.

As far as acro - I suppose that's always a good thing to have ruled out. Others will have to weigh in on that - those who have acro cats - like @Olive & Paula or @Wendy&Neko or @Amanda and a Loudogg could comment perhaps on an assessment.

I hope that the visit is enriching and productive!

I have been so fortunate to have worked remotely for 11 years as of Jan 1. I never thought it would last this long and I love it! I was getting ready to retire when this opportunity came up. I love the work I do although some days are - shall we say - more trying on the spirit than others. Sometimes it's downright difficult to work at home - all alone - other times I couldn't be happier! I miss the social interactions - however get a great deal more work done than I ever did when I was in the office. So many interruptions there! I sure don't miss the commuting at all! And I'm very happy that I get to be a 'stay at home' cat mom to Luci - never imagined I have to check the cat a zillion times a day!

It's to early to consider acro. Her dose really needs to climb up there. If she must have dry food try switching that Young Again in more than the others. Or crushing a little of the dry on top of the wet food.
 
Good luck with the vet visit today. I would be interested in hearing what an endocrinologist has to say. We have seen one or two other members see one, one I was not impressed with. Always good to know if there is a knowledgeable one around.

Yes and no. There have been acromegalic cats on doses as small as 1 unit. Neko was on less than one unit for a while, but that was after treatment. It’s not really a high dose condition - I don’t like that name. However, there can also be many reasons a relatively newly diagnosed cat on small doses isn’t regulated yet, so it’s not something we would suggest you test for yet. Especially with dry food in the picture. Plus, the research out of the Royal Veterinary Schools Diabetes Remission Clinic is that you not test for acromegaly until after at least 73 days after starting insulin. Dr. Woolhead found that there was one third false negatives if tested too early. It is too early for Chanel. Was there a reason you or your vet has mentioned acromegaly?
 
I am STUPID. I wanted to try brush her teeth and now looking at the label, I see dextrose...so if her BG later tonight is in 500s...yup, that is on me.
Visit at the vet. Nice, knowledgeable. She said we are on the right path with insulin but definitely we need to increase the dose. Recommended to get her off dry food. Also not convinced that Chanel needs a T4 test just yet. However, not convinced that the previous vet in Chelmsford did the right test for pancreatitis, her opinion is that Chanel actually had pancreatitis. I guess we will never know.
The most important thing is that Chanel has gingivitis, her gums are again reddish. Do you guys know any toothpaste that is recommended for diabetic cats?
 
Are you planning on getting a dental? Bad teeth/gums get in the way of many a cat and regulation. My vet said just to wipe the teeth with gauze or a baby facecloth. No toothpaste needed.

She just had a dental on October 21. I am planning on wiping the teeth with gauze or baby facecloth like you said. I know dental cleaning is important but the previous one was so soon and we are still dealing with the aftermath of that or should I say Chanel still does...
After going to the vet today, the little was was so stressed (they noticed yet again the breathing, oh well) that she got hungry. We came home and she was staring at the dry food but not willing to eat it...and then the wet food appeared and Chanel was all over it...the entire can.
One thing the doctor said and I agree..Chanel needs to lose weight, she is now 7 kg (15.4 LBS). Her heaviest was 18 LBS and after the dental cleaning she was 14.74 LBS. We need to get the baby to exercise.
