Soo... I am assuming we have some NDW going on. She is acting her usual but I do think that the lantus makes her muscles twitchy. Anyone else ever see this with their kitty?? It was quite evident about an hour after her pm dose. It’s been there ever since we started this journey. Thoughts??

Some kitties react to the acid base of Lantus. It’s especially noticeable in high dose cats, but I have seen it on some low dose as well. @Ti-Mousse and @Marje and Gracie are two such examples. One option could be to switch to Levemir. The action is similar to Lantus, but onset and nadir are later, onset around +4 and nadir around +8. The late nadir can be an issue for some. It was better for me, cause Neko most of the time nadired even later. I didn’t think Lantus bothered Neko much, but she would occasionally walk away during the shot. She purred through Lev shots.

So if the nadir is later, how is it in comparison to lantus with overlap and bounce?? Is it just as touchy with dose changes? Would you then work in a regular insulin as well to take care of the immediate food glucose spike?

For me and Neko, Lev just shifted a couple hours when things happened in the cycle. There is some anecdotal evidence that Lev may have better duration and overlap. Often, but not always, cats will flatten out on Lev. We dose it the same way as Lantus, but with later nadirs, you have to get used to shooting lower. Neko regularly nadired at preshot time, but I then had 5 hours until onset so plenty of time for her to rise. I did spot a nadir at +15 a couple times.

Some people use R with Lev, but more for high dose cats or those with ketone prone kitties. I did use R, but not to combat a food spike which was typically only around 30 points.

Gracie felt a sting at 1u. I did switch to Levemir and wished I had used it from day one.

Lantus typically nadirs at +6 (ECID). Lev +8 - +12. Onset is about +4. I found Gracie had much better carryover and overlap with Levemir. Some cats quit bouncing; Gracie didn’t but her bounces were minimized for the most part.

I’m not sure what you mean by “work in a regular insulin” to take care of food glucose spikes. If you mean shoot a bonus insulin, it’s not necessary unless a kitty is high dose or bounces a lot. I did use R with Gracie but mainly to be a little more aggressive with getting and keeping her in normal numbers.

Levemir is long-lasting insulin. I found shooting a nadir at +12 to be very beneficial to keep her in better numbers longer.

Hmmm... something to think about if she isnt responding as nicely to lantus as I had hoped. So I take it, Levemir is preferred more so than pzi with its effects on cats?? And should I decide to switch down the line, would I with hold from any insulin for a day for a washout of one before starting or that isn’t necessary? How is Levemir dosed? Thanks!

Some cats, and this is all ECID, have a strong reaction to the switch between the L insulins. So we suggest starting the second L at a dose of 70% of the first. That's a safety thing. Some cats need more, or less, or the same dose of Lev as they did Lantus. We shoot Lantus one cycle, Lev the next but at the reduced dose. As above, you can follow either TR or SLGS with Levemir.

I have seen several kitties go from Prozinc to Levemir, not the other way around.

I started Ti-Mousse with Lantus and right at the beginning he hated shots as if it was so painful and he also had muscle twitching! After a month I switched him to Levemir and no more problems ever! Shot time has always been easy after! Good luck for little Charlotte!

So have you found that the degree of bouncing is less?? I find the lantus to be sooo touchy and somewhat unpredictable. Do you seem to have more control with the numbers stating in range? Thanks

I found that Neko bounced less with Lev, but it is ECID. Our second Lev cycle was all blue. I did find it more predictable, but I also was getting more experienced at figuring out her cycles and Neko was learning to bounce a tiny bit less even on Lantus.
