My orginal script says to give Mr.F 3 units Q12hrs, his VERY 1st dose of insulin I gave him 3u, after talking with other members it was suggested that, that dose was too high based on his weight (11.5#) so the next dose i gave 1unit and I have continued that until tonights pmps where i uped it to 2u. He's been in the 300's since i started the 1unit dose, he only got down to 180 after the 3unit dose.

My question is.... 1. Am I doing this right? 2. How do i know when I need to increase/decrease insulin dose?

My biggest fear is him going hypo when I'm gone at work or sleeping at night.

Not exactly....We never increase by whole units. Going from 1U to 2U is a 100% increase in dose. That's like a human going from 20U to 40U. We only recommend doing dose increases in .25 to .5 unit increments.

If you're doing SLGS, you hold the dose for 7 days before changing the dose and you need to do a curve first (test every 2 hours for 12 hours or every 3 hours for 18)

Do you have syringes with half unit markings? There's no such thing as a syringe with .25 unit marks, so the half unit ones are the best we can do. We just have to eyeball the .25 and .75 doses.

2. How do i know when I need to increase/decrease insulin dose?

You know when you do your curve (or at least get more data by testing at different points in the cycle)

None of us were experts in dosing and not everybody is comfortable giving dosing advice, but if you post daily with updates, people will start to watch for you and let you know when it's time to go up.

Now if he were to drop below 90 at any time, he'd earn a reduction in dose (on SLGS)

How many cycles has he been getting Lantus? Did he get his first dose on the PM cycle on 10/28?
